Physician is responsible for providing medical diagnosis, treatment services, and managing quality health care services to patients of Aviva Health. Provides quality of care to patients, consistent with his/her training and expertise, and appropriate to the ambulatory care setting provided. Acts as consultant for and provides guidance for Advanced Practice Providers regarding clinical practice issues.</span></p>\n<p><br></p>\n<p><span style=\"font-family: Inter, sans-serif; font-size: 12pt; font-weight: bold\">ESSENTIAL FUNCTIONS:</span></p>\n<ul>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Provides high quality primary care to Aviva Health patients of neonatal, pediatric, and adolescent groups.</span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Leads cross-functional care teams.</span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Provides appropriate Specialists referrals and follow-up.</span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Partners with Aviva Health’s Mental Health team and resources.<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Engages patient, family, and cross-functional care teams in sustained health and well-being of patient.<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Maintains electronic health record (EHR) in compliance with Aviva Health standards with high efficiency.<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Maintains familiarity with ongoing research and development in relevant fields.<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Commitment to a team approach to health care delivery.<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Participates in peer and chart review process.<br></span></li>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Performs other related and/or necessary tasks to achieve clinic goals and objectives, as directed by the Chief    Medical Officer.<br></span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: Inter, sans-serif; font-size: 12pt; font-weight: bold\">QUALIFICATIONS:</span><br></p>\n<ul>\n<li><span style=\"font-family: Inter, sans-serif; font-size: 12pt\">Completion of medical degree program. Doctor of Medicine (MD) or Osteopathy (DO). Completion of three-year pediatrics residency.
