In this role, you&#39;ll build relationships with physicians, hospitals, case managers, skilled nursing facilities, and other referral partners while serving as a trusted resource for home health solutions.</p><p>If you have a passion for healthcare sales, relationship building, and helping patients receive the care they need, we&#39;d love to meet you.</p><h3>What You&#39;ll Do</h3><ul data-spread=\"false\"><li>Develop and maintain referral relationships with healthcare providers and community partners</li><li>Promote Choice Health at Home as the preferred provider for home health services</li><li>Identify new business opportunities and referral sources</li><li>Collaborate with clinical and operational teams to support patient admissions and care transitions</li><li>Educate referral partners on services, programs, and patient eligibility</li><li>Ensure compliance with all federal, state, and local marketing regulations</li><li>Participate in community outreach, networking events, and professional organizations</li><li>Meet and exceed monthly admission and growth goals</li><li>Provide market feedback and recommendations for service expansion</li></ul><h3>What We&#39;re Looking For</h3><ul data-spread=\"false\"><li>2+ years of home health or hospice sales experience preferred</li><li>Strong relationship-building and communication skills</li><li>Experience using CRM systems preferred</li><li>HomeCare HomeBase experience preferred</li><li>Valid driver&#39;s license and auto insurance required</li><li>Proven ability to meet sales and referral growth goals</li></ul><h3>Benefits &amp;
