The Patient Navigator provides field-based case management services to patients enrolled in the HIV care coordination program. Patient Navigator is the main point of contact for the patient. The Patient Navigator builds strong relationships with HIV positive patient in order to help them connected and to stay engage in prevention and treatment services. &nbsp;Patient Navigator is committed to removing the patient&rsquo;s barrier to care by identifying critical resources and helping them navigate through health care services and system and to promote patients&rsquo; health.
