Description
Overview
Nurse Practitioner (NP)
Samaritan Daytop Village is a leading human services organization in New York State serving over 38,000 New Yorkers annually across more than 60 locations through Transitional Housing, Community Based Services, and Residential and Outpatient programs. First Steps Intensive Crisis Stabilization Center (ICSC), operated by Samaritan Daytop Village in Rockland County, is a 24/7 program dually licensed by OMH and OASAS to provide rapid, short-term support for children ages 5+ and adults in mental health or substance use crisis. Our multidisciplinary team utilizes a trauma-informed, harm-reduction framework to stabilize individuals and connect them with long-term care while avoiding unnecessary emergency room visits.
We are seeking a Nurse Practitioner (NP) to function as an advanced practice provider delivering medical evaluation, physical health stabilization, and integrated primary care to individuals in a behavioral health setting. Practicing within the full scope of New York State licensure, the NP ensures that clients experiencing acute mental health or substance use emergencies are medically cleared and treated for underlying health conditions. This role is a vital part of a 24/7 multidisciplinary team, bridging the gap between physical health and behavioral health crisis intervention.
Responsibilities
Performs comprehensive physical health evaluations, including medical histories, physical exams, and health risk assessments
Manages acute and chronic medical conditions for clients admitted for mental health or substance use stabilization
Prescribes and manages medications for physical health needs and provides support for withdrawal management protocols

Collaborates with the medical director, psychiatrists, RNs, and social workers to develop integrated discharge and safety plans
Participates in multidisciplinary rounds to ensure clients are medically stable for behavioral health programming
Maintains accurate and timely documentation in the Electronic Health Record (Avatar) in compliance with OMH/OASAS and HIPAA standards
Adheres to DEA, PMP/I-STOP, and EPCS requirements for all prescription protocols
Provides health education to patients and families regarding medical diagnoses, harm reduction, and overdose prevention
Engages in peer reviews and quality improvement initiatives to maintain high standards of integrated care
Qualifications
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited Nurse Practitioner program
Active, unrestricted New York State Registered Professional Nurse license
Active, unrestricted New York State Nurse Practitioner certification (Family, Adult, or Gerontology)
Active DEA registration with authority to prescribe controlled substances
Minimum of one (1) year of experience as a Nurse Practitioner (NP) preferred; new graduates with strong clinical backgrounds considered
Experience in emergency medicine, urgent care, or behavioral health settings strongly preferred
Current CPR/BLS certification; ACLS preferred
Advanced clinical knowledge of medical stabilization and the physical impacts of substance use or withdrawal
Expert assessment skills for identifying medical emergencies masked by behavioral health symptoms
Familiarity with trauma-informed, recovery-oriented, and harm-reduction clinical practices
Proficiency with Electronic Health Records (Avatar preferred) and EPCS requirements
Excellent collaborative communication skills for working with a diverse team of medical and non-medical professionals
Additional Information
This is a full-time exempt role with an annual salary of $150,000-$169,744 with a full benefits package. Benefits include medical, dental, and vision insurance, 25 days of paid time off, access to a retirement account with an employer to match, and tuition reimbursement.
