As a clinical leader within our primary care setting, you will provide effective, efficient, and appropriate services to our members while demonstrating safe clinical decision-making and expert care. You will work collaboratively with our clinical practice team to deliver exceptional care that aligns with our mission of transforming healthcare through our payvider model.</p><h2>POSITION DUTIES & RESPONSIBILITIES</h2><ul><li>Interview members to obtain medical history, perform physical examinations, order laboratory and other diagnostic tests, prescribe medications, and recommend treatments</li><li>Provide continuity in managed care for members with pre-existing long-term conditions</li><li>Make referrals for secondary and tertiary care as needed in collaboration with the care team</li><li>Perform medical procedures according to issued privileges</li><li>Provide health maintenance visits, evaluate immunization needs, offer anticipatory guidance and referrals</li><li>Perform contract and special physical examinations for employment and other purposes</li><li>Conduct STD screening, family planning, and health education</li><li>Document and code all member contacts accurately and legibly in the electronic medical record</li><li>Function as an active member of the medical team</li><li>Participate in and manage care team meetings</li><li>Contribute to organizational initiatives, including participation on clinical committees, providing education, quality review, and other mission-aligned activities</li></ul><h2>DESIRED PROFESSIONAL SKILLS & EXPERIENCE</h2><h3>Required:</h3><ul><li>Graduate of an accredited Physician Assistant Program and certified by the National Commission on Certification of Physician Assistants (NCCPA) or Nurse Practitioner Program and certified by the American Nurses Credentialing Center or the American Academy of Nurse Practitioners</li><li>Current licensure as a PA/NP to practice in the State of Texas</li><li>Licensed and certified to practice in Family Medicine</li><li>Current DEA and CPR certification</li><li>Valid Texas Driver's License</li><li>Ability to meet and maintain current organizational credentialing and privileging requirements</li><li>Comfortable working as a solo provider in a clinic team setting</li><li>Computer literacy with ability to navigate electronic health record systems</li><li>Excellent customer service skills and professional communication</li><li>Ability to interact effectively with persons from diverse backgrounds</li><li>Strong teamwork capabilities while maintaining ability to function independently</li><li>Sound judgment, flexibility to shift priorities, and commitment to confidentiality</li></ul><h3>Preferred:</h3><ul><li>2+ years' experience as an Nurse Practitioner or Physician Assistant in a primary care setting</li><li>Bilingual in English/Spanish</li></ul><h3>Physical Requirements:</h3><ul><li>Ability to work irregular hours in a moderate-stress environment</li><li>Capacity to handle exposure to blood, body fluids, communicable diseases</li><li>Ability to push/pull heavy objects and lift up to 50 pounds</li><li>Full range of body motion for patient care activities</li></ul><h2>WHAT WE OFFER</h2><ul><li>Competitive salary and incentives</li><li>Generous PTO</li><li>10 paid holidays</li><li>CME Allowance and time off</li><li>Medical, Dental and Vision Insurance</li><li>Occurrence-based malpractice</li><li>401k Investment Plan</li><li>Company Equity</li></ul><p>At Harbor Health, we're transforming healthcare in Texas through collaboration and innovation.
