The ideal candidate will be results-driven, team-and detail-oriented professional who is comfortable working on multiple tasks in a deadline-oriented environment.
Responsibilities : To provide specialized services to clients who are chemically dependent and/or their significant others. Provide and coordinate substance abuse assessment, education, training, and placement services for clients of Community Support Services programs .
Conduct initial substance abuse assessment services to all clients on team. Provide individual, group, and family substance abuse treatment and counseling. Coordinates referrals to other substance abuse treatment programs or facilities in the community/state, i.e. medically assisted treatment or inpatient. Provide chemical dependency consultation, education and training to team personnel. Develop and implement treatment plans and document services consistent with Center standards. Coordinate discharge planning, follow up and aftercare. Monitor signs and symptoms of psychiatric functioning; medication regimen, and substance use. Assist in community networking and education. Provide assistance with income maintenance, living arrangements. Provide education, training and modeling in basic daily living skills, social or parenting skills. Teach clients effective techniques for managing their medication as prescribed. Coordinate services within the Center as needed. Assist clients in developing and strengthening their support network to accomplish goals and objectives. Complete billing forms, logs, data collection forms and clinical documentation in accordance with Center standards. Requirements:
Bachelor Degree in behavioral health field or equivalent combination of education and experience required. Two (2) years of relevant substance abuse experience. CDP or working toward credentialing. LCDP preferred. Valid driver’s license required. Benefits:
