Provide and coordinate substance abuse assessment, education, training, and placement services for clients of Community Support Services programs<strong>. &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</strong></p><ul><li><strong>&nbsp;</strong>Conduct initial substance abuse assessment services to all clients on team.</li><li>Provide individual, group, and family substance abuse treatment and counseling.</li><li>Coordinates referrals to other substance abuse treatment programs or facilities in the community/state, i.e. medically assisted treatment or inpatient.</li><li>Provide chemical dependency consultation, education and training to team personnel.</li><li>Develop and implement treatment plans and document services consistent with Center standards.</li><li>Coordinate discharge planning, follow up and aftercare.</li><li>Monitor signs and symptoms of psychiatric functioning; medication regimen, and substance use.</li><li>Assist in community networking and education.</li><li>Provide assistance with income maintenance, living arrangements.</li></ul><ul start=\"10\" type=\"1\"><li>Provide education, training and modeling in basic daily living skills, social or parenting skills.</li><li>Teach clients effective techniques for managing their medication as prescribed.</li><li>Coordinate services within the Center as needed.</li><li>Assist clients in developing and strengthening their support network to accomplish goals and objectives.</li><li>Complete billing forms, logs, data collection forms and clinical documentation in accordance with Center standards.</li></ul><p><strong>Requirements:</strong></p><ul><li>Bachelor Degree in behavioral health field or equivalent combination of education and experience required.</li><li>Two (2) years of relevant substance abuse experience.</li><li><span style=\"font-size: 14px;\">CDP or working toward credentialing. LCDP preferred.
