The Licensed Vocational/Practical Nurse reports to the Hospice Administrator.</p><p><br></p><p><strong>Role Expectations:</strong></p><ul><li>Under the direction of the RN, assists in identifying the patient’s physical, psychosocial, and environmental needs as evidenced by documentation clinical record, team report, and evaluations.</li><li>Participates in planning and implementing care in conjunction with the RN, delivers and documents care based on the Interdisciplinary plan of care&nbsp;&nbsp;</li><li>Provides nursing care utilizing infection control measures (OSHA)</li><li>Assures the continuity of care through delivery of quality patient care.</li><li>Work with the RN and the Interdisciplinary Group/Team to ensure patient/family needs are met.</li><li>Communicate with attending physicians, hospice physicians, and other physicians involved in the patient’s care.</li><li>Timely submission of all required paperwork.</li><li>Demonstrates commitment and professional growth by participating in hospice sponsored in-service programs and maintaining or improving competency.
