Your expertise, clinical judgment, and strong interpersonal skills will be crucial in delivering high-quality care and ensuring positive patient outcomes.</span></p>\n<p><br></p>\n<p><span style=\"font-family: helvetica; font-size: 14px; font-weight: bold\">Responsibilities:</span></p>\n<p><br></p>\n<ol>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Patient Assessment and Care Planning:</span>\n<ul>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Conduct comprehensive assessments of patients' physical, emotional, and social needs to develop individualized care plans.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Collaborate with patients, their families, and healthcare teams to establish goals and interventions to address patients' health conditions and promote self-management.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Regularly reassess patients' conditions, monitor vital signs, and evaluate the effectiveness of interventions, adjusting care plans as necessary.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Educate patients and their families on disease management, medication administration, and self-care techniques.</span></li>\n</ul>\n</li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Skilled Nursing Care:</span>\n<ul>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Provide skilled nursing care including wound care, medication management, IV therapy, catheter care, and other treatments as prescribed.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Administer medications and treatments accurately and safely, following established protocols and guidelines.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Perform skilled procedures such as venipuncture, tracheostomy care, and respiratory therapy.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Monitor and manage symptoms, pain, and comfort levels, implementing appropriate interventions to alleviate distress.</span></li>\n</ul>\n</li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Coordination and Collaboration:</span>\n<ul>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Collaborate with physicians, therapists, social workers, and other healthcare professionals to develop and implement comprehensive care plans.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Coordinate with the interdisciplinary team to ensure continuity of care and effective communication.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Communicate regularly with patients, families, and caregivers to provide updates on the patient's condition, progress, and any changes to the care plan.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Coordinate and supervise home health aides and other support staff involved in patient care.</span></li>\n</ul>\n</li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Documentation and Compliance:</span>\n<ul>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Maintain timely, accurate and thorough documentation of assessments, care provided, and patient outcomes using Nestmed AI charting tools and Kinnser EHR system. </span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Ensure compliance with all regulatory requirements, agency policies, and standards of care.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Complete and submit clinical documentation, including OASIS assessments, within specified timeframes.</span></li>\n</ul>\n</li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Quality Improvement:</span>\n<ul>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Participate in quality improvement activities, including case conferences, chart reviews, and performance improvement initiatives.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Identify areas for improvement in care delivery and actively contribute to the development and implementation of best practices.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Stay updated with the latest evidence-based practices and industry trends related to home health nursing.</span></li>\n</ul>\n</li>\n</ol>\n<p><br></p>\n<p><span style=\"font-family: helvetica; font-size: 14px; font-weight: bold\">Requirements:</span></p>\n<p><br></p>\n<ul>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Current and unrestricted Registered Nurse (RN) license valid in the State of Texas. </span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Minimum of one year of nursing experience in home health or related setting.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 10pt\">Proficiency in using EHR (Kinnser/WellSky a plus) systems, Nestmed or other AI charting and other apps such as Slack. </span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Knowledge of home health regulations, Medicare/Medicaid guidelines, and OASIS documentation. </span><br></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Strong clinical assessment and critical thinking skills to effectively address patients' complex needs.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Excellent communication and interpersonal skills to establish rapport with patients, families, and healthcare professionals.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Ability to work autonomously, prioritize tasks, and manage time efficiently in a dynamic home health environment.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">Willingness to travel to patients' homes within the designated service area and have reliable transportation.</span></li>\n<li><span style=\"font-family: helvetica; font-size: 14px\">BLS (Basic Life Support) certification.</span><br></li>\n</ul>\n<p><span style=\"font-family: helvetica;
