    "description_html": "<p><strong>Company: IEP - Post Acute Care (Jackson, MI)</strong></p><p><br></p><p><strong>Job Description: Advanced Practice Provider</strong> (<strong>Nurse Practitioner or Physician Assistant)</strong></p><p>The Certified Nurse Practitioner (Nurse Practitioner), in collaboration and consultation with physicians, Care Coordinators, staff RNs, and other healthcare professionals, provides holistic, compassionate care to <strong>post-acute care facilities</strong>. The Nurse Practitioner practices within the context of collaborative management with a physician(s) in diagnosing, managing, and preventing acute and chronic illness and disease, and promoting wellness. <strong>Work hours: TBD</strong></p><p><strong>Essential Duties and Responsibilities</strong></p><p>The essential functions include, but are not limited to the following:</p><ul><li>Performs in-depth health risk assessments on all assigned patients</li><li>Works collaboratively with physicians, Care Coordinators, facility staff, family members, key caregivers, and any appropriate ancillary medical personnel as appropriate</li><li>Responsible for identifying overall quality improvement activities through assessing and planning health care which is individualized to the member and consistent with policies and protocols</li><li>Reviews and interprets the medical, surgical, and social history of members</li><li>Performs appropriate screening procedures for members</li><li>Interprets laboratory and other diagnostic data and recommends appropriate treatments or actions for follow-up, reconciles medications</li><li>Responsible for prescribing or performing various treatment modalities - including, but not limited to medications, injections, wellness counseling, suturing, and ancillary testing</li><li>Participates in all mandatory compliance and other training programs, seeks guidance for compliance-related concerns, and adheres to all applicable laws, regulations, policies, and procedures</li><li>Refers members to PCP, Care Management, and Disease Management programs as necessary</li><li>Provides constructive and timely feedback about recommendations to improve and/or streamline operations.</li><li>Maintain a focus on timely customer service.</li><li>Perform direct Care Management Support as needed.</li><li>Synthesizes information and easily diagnoses and participates in the development of the Plan of Care.</li><li>Creates positive rapport and engagement with plan members</li><li>Assists on special projects as assigned</li><li>Participates in client meetings as requested</li><li>Communicates and collaborates with Facility leadership to achieve Member care needs and goals.</li></ul><p><strong>Minimum Qualifications (Knowledge, Skills, and Abilities)</strong></p><ul><li>Due to the nature of our business, we require all of our employees to be fully vaccinated within 90 days of their start date, in compliance with all applicable EEOC guidance regarding vaccines and qualified exemptions.</li><li>Must be able to demonstrate computer skills, including the use of e-mail and Microsoft office word and Excel</li><li>Must be able to demonstrate effective written communication skills including but not limited to writing reports and correspondence</li><li>Must be able to demonstrate effective verbal communication skills, including but not limited to speaking effectively to members, providers, families, and staff and listening to members, providers, and families</li><li>Demonstrated ability to meet deadlines, and use time effectively and efficiently.</li><li>Demonstrated successful experience working in an environment of change. Effectively copes with change and uncertainty; responds and \"shifts gears\" comfortably</li><li>Is dedicated to meeting the expectations and requirements of both external and internal customers, and acts with customers in mind</li><li>Demonstrated ability to establish and maintain effective relationships with customers, and gain their trust and respect</li><li>Uses logical methods to solve problems with effective solutions; looks beyond the obvious for solutions</li><li>Is action oriented and sees challenges as opportunities</li></ul><p><strong>Baseline Clinical Knowledge</strong></p><ul><li>Minimum 1 years of clinical experience as a Nurse Practitioner/Physician Assistant&nbsp;</li><li>Proven ability to perform comprehensive, adult/geriatric physical exam</li><li>Demonstrated ability to identify, diagnose and document chronic disease in adults</li><li>Demonstrated ability to recognize conditions, characteristics, and behaviors that place seniors at risk</li><li>Proven ability to pursue feedback, continuing education, and professional development</li><li>Ability to effectively reconcile medications</li><li>Demonstrated ability to identify and document the needs of clinically complex Members</li><li>Pertinent Clinical Work Experience which is relevant to the population being served.</li></ul><p><strong>Physical Demands and Work Environment</strong></p><p>The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this position.
