    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><div><p id=\"isPasted\"><strong>Hours</strong>: Full-time &ndash; 37.5 hours per week (Monday through Friday, 8:30 a.m. &ndash; 5:00 p.m.)</p><p><strong>Location</strong>: Lynchburg VA, 24501</p><p><em>Compensation dependent on experience&nbsp;</em></p><p><strong><u>Position Summary</u></strong></p><p>The Case Manager promotes and supports individuals who are at risk of developing or who have mental health and/or substance use issues. Services include assessing, monitoring, and linking to services, supportive level counseling, education, and advocacy. &nbsp;Case management services may also be provided to a small number of priority population women with substance use disorders as needed.</p><p><strong><u>Required Education and Experience</u></strong></p><ul><li>Bachelor&rsquo;s degree from an accredited college or university in an applicable human services field; or relevant education which indicates that the individual possesses the required knowledge, skills, and abilities.</li><li>A minimum of one (1) year of relevant mental health, developmental disability, and/or substance use work experience with the identified population, or a combination thereof that demonstrates that the candidate possesses the required knowledge, skills, and abilities; and/or an equivalent combination of education, training, and experience that provides the individual possesses the required knowledge, skills, and abilities.</li></ul><p><strong><u>Related Knowledge, Skills and Abilities</u></strong></p><ul><li>Services, systems, and programs available in the community including primary health care, support services, eligibility criteria and intake processes, generic community resources, and mental health, developmental disabilities, and substance abuse treatment programs.</li><li>The nature of serious mental illness, developmental disabilities, and substance abuse depending on the population served, including clinical and developmental issues.</li><li>Different types of assessments, including functional assessments, and their uses in service planning.</li><li>Treatment modalities and intervention techniques, such as behavior management, independent living skills training, supportive counseling, family education, crisis intervention, discharge planning, and service coordination.</li><li>The service planning process and major components of a service plan.</li><li>The use of medications in the care or treatment of the population served.</li><li>All applicable federal and state laws, state regulations, and local ordinances.</li><li>Identifying and documenting an individual&#39;s needs for resources, services and other supports.</li><li>Using information from assessments, evaluations, observation, and interviews to develop individual service plans.</li><li>Identifying services and resources within the community and established service system to meet the individual&#39;s needs; and documenting how resources, services, and natural supports, such as family, can be utilized to achieve an individual&#39;s personal habilitative/rehabilitative and life goals.</li><li>Coordinating the provision of services by public and private providers.</li><li>Work as team members, maintaining effective inter- and intra-agency relationships.</li><li>Work independently, performing position duties under general supervision.</li><li>Engage and sustain ongoing relationships with individuals receiving services.</li><li>To drive and must possess a valid Virginia driver&rsquo;s license. May be called upon to travel to any Horizon worksite; transport clients, provide services to clients at other locations.
