In this role, you’ll provide life-changing care to patients throughout the continuum of recovery - from initial stabilization to long-term maintenance.</p><p><br></p><p><strong>Responsibilities include:</strong></p><ul><li>Conduct new and established patient assessments, and develop individualized treatment plans for patients with substance use disorders, including opioids, alcohol, stimulants, and sedatives</li><li>Initiate and manage patients on buprenorphine and other maintenance medications for substance use disorders</li><li>Administer long-acting injectable medications (buprenorphine, naltrexone)</li><li>Monitor response to treatment, adjust medications, and review lab results</li><li>Collaborate with behavioral health and care coordination teams</li><li>Document appropriately in our EMR (eCW)</li><li>Deliver care both in-person and via telemedicine</li></ul><p><br></p><p><strong>Join us in making a difference — one patient, one family, and one community at a time.</strong></p>",
