Join a place where you can build your career while making a difference and enjoying the balance you need for a full life. </p>\n<h2>Responsibilities</h2>\n<ul>\n <li><p>Conduct comprehensive patient assessments, including history taking and physical examinations across assigned clinic sites. </p></li>\n <li><p>Diagnosis and manage acute, chronic, and preventative health conditions in alignment with evidence based guidelines. </p></li>\n <li><p>Develop and implement patient care plans, including ordering and interpreting diagnostic tests, prescribing medications and initiating referrals as deemed appropriate. </p></li>\n <li><p>Maintain accurate, timely, and thorough documentation in the electronic medical record (Cerner). </p></li>\n <li><p>Collaborates with providers, care coordinators, medical assistants, and other interdisciplinary team members to ensure seamless care delivery. </p></li>\n <li><p>Participate in shared call with primary care providers.</p></li>\n</ul>\n<h2>Qualifications</h2>\n<ul>\n <li><p>Completion of Masters in Nursing or Doctor of Nursing Practice from an accredited program </p></li>\n <li><p>Minimum of 2-3 years of Nurse Practitioner experience within a primary care setting </p></li>\n <li><p>Strong clinic assessment, critical thinking, and decision making skills</p></li>\n <li><p>Excellent communication and interpersonal skills with patients and care team members</p></li>\n</ul>\n<p><strong> </strong></p>\n<p>The above statements are intended to describe the functions and related requirements of persons assigned to this job.
