    "requisitionDescription": "<div><div><div><div><div><div><div><div><div><div><div><div><div><p><strong>POSITION PURPOSE:&nbsp;</strong><br>Licensed Therapist (LMHC, LCSW, LMFT) CONTRACT&nbsp;&nbsp;&nbsp;is responsible for providing individual and group counseling as needed. Therapist uses a holistic approach to ensure that clients therapeutic issues are being addressed and makes referrals for other issues that client may have as they present.<br><br><br><strong>POSITION REQUIREMENTS / QUALIFICATIONS:</strong><br><br><strong><ins>Education/Experience: &nbsp; &nbsp;</ins></strong><br>Graduated from an accredited university with a Bachelor&rsquo;s Degree in Psychology, or Social Work and a Master&rsquo;s Degree in Social Work, or Mental Health Counseling or related field.<br>&nbsp; &nbsp;&nbsp;<br><strong><ins>Licensure / Certification</ins></strong>: &nbsp; &nbsp;<br>Maintain current CPR certification from the American Heart Association. Licensed as a Mental Health Social Worker or Mental Health Counselor in the state of Florida. Must have a valid Florida Driver&rsquo;s License.<br>&nbsp; &nbsp;&nbsp;<br><strong><ins>Skills / Ability:</ins></strong>&nbsp; &nbsp;&nbsp;<br>Proficiency in verbal and written skills. Must be computer literate. Possesses knowledge of FCBS, BHRS computer system. Administers behavioral rating scale, client certification and Mental Health Outcome Measures Input Form, Target Population Enrollment Form, FARS and documentation forms. Possesses knowledge of Confidentiality and Patient Rights.<br><br><br><strong>POSITION RESPONSIBILITIES (THIS IS A EXEMPT POSITION)</strong><br><br>Provides care primarily for the following groups:  Infant  Pediatric  Adolescent  Adult  Geriatric<br><br>Provides high quality of services at all times in an outpatient Behavioral Healthcare component, with emphasis on age appropriate, bio-psychosocial issues.<br><br>Provides counseling services to adult and adolescents and their families.<br><br>Provides consultation to clinicians, primary health physicians, and other staff with the BHCC.<br><br>Maintains a daily log of productivity and submits a monthly statistical report to clinical supervisor.<br><br>Facilitates daily, weekly education and counseling groups<br><br>Conducts psychosocial assessments on patients admitted.<br><br>Participates in daily treatment team activities as a core treatment team member.<br><br>Coordinates discharge plans.<br><br>Participates in in-patient and family education.<br><br>Monitors client&rsquo;s ongoing needs, goals and progress.<br><br>Understands and applies knowledge pertaining to Community Health of South Florida, Inc.
