Integrates evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered, promoting quality and efficiency in the delivery of post-acute care. Represents the area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.</p>\n<h2>Qualifications</h2>\n<p>Education and experience, essential</p>\n<ul>\n <li>Must be a graduate of an approved school of nursing, therapy or social work. </li>\n <li>Must be licensed in the state where they currently practice. </li>\n <li>Must have two years' demonstrated field experience. </li>\n <li>Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.</li>\n <li>Must have basic demonstrated technology skills, including operation of a mobile device.</li>\n</ul>\n<p>Education and experience, preferred</p>\n<ul>\n <li>A registered nurse or physical therapist is preferred. </li>\n <li>Three years of field experience is preferred.</li>\n <li>Previous experience in home health or healthcare sales is preferred.</li>\n</ul>\n<p>Requirements</p>\n<ul>\n <li>Must possess a valid state driver license</li>\n <li>Must maintain automobile liability insurance as required by law</li>\n <li>Must maintain dependable transportation in good working condition</li>\n <li>Must be able to safely drive an automobile in all types of weather conditions</li>\n</ul>\n<h2>Additional Information</h2>\n<p><em>Enhabit Home Health & Hospice is an equal opportunity employer.
