As part of a broader community vision to create a continuum of care, Guelph CHC's HART hub team will provide health care, support, connect, refer and engage in shared care planning for HART Hub clients, with the goal of providing wrap-around, stage-matched supports for clients.</span></span><span> </span></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span><span>PRINCIPAL JOB DUTIES</span></span></span><span> </span></p>\n<ul>\n<li><span><span>Identify</span><span><span> </span>client-centred goals that lead to outcomes that are meaningful and relevant to the individual</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Use a trauma-informed</span><span><span> </span></span><span>approach</span><span> to</span><span><span> </span>deliver stage matched interventions</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Collaborate with other provider types within the Hub team to provide wraparound care</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Support clients in navigating social<span> </span></span><span>assistance</span><span>, housing, food security,<span> </span></span><span>transportation</span><span><span> </span>and other needs as pertains to their well-being</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Support the<span> </span></span><span>development and<span> </span></span><span>implementation of<span> </span></span><span>care plans that address client’s health, basic needs, mental health substance use</span><span>,<span> </span></span><span>recovery</span><span><span> </span>and<span> </span></span><span>treatment</span><span><span> </span></span><span>needs</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Maintains client records, assessment<span> </span></span><span>notes</span><span><span> </span>and correspondence within the safeguard of confidentiality</span></span><span> </span></li>\n</ul>\n<ul>\n<li><span><span>Provide accompaniment and advocacy to ensure clients can access the care they need</span></span><span> </span></li>\n<li><span><span>Liaise with other care providers in the system<span> </span></span><span>(e.g. Hospital, primary care, outreach services)<span> </span></span><span>to<span> </span></span><span>support coordinated<span> </span></span><span>care for</span><span><span> </span>clients</span></span><span> </span></li>\n<li><span><span>Participate in community groups and committees relating to the development and delivery of health care and social services in the community</span></span><span> </span></li>\n</ul>\n<p><span style=\"font-weight: bold\"><span><br></span></span></p>\n<p><span style=\"font-weight: bold\"><span><span>KNOWLEDGE, SKILLS &amp;
