This role provides patient centric care and is primarily responsible for nursing assessment and diagnosis, planning, implementation, and evaluation.</p>\n<p> </p>\n<p>This position is 0.6 FTE, 24 hours/week.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changes</li>\n <li>Develops a plan of care based on the patients’ disease, symptoms, and response to treatment</li>\n <li>Educates patients, families, and caregivers on disease processes, treatment, side effects and symptom management</li>\n <li>Coordinates with the interdisciplinary team to ensure continuity of care</li>\n <li>Refers patients, families, and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc.</li>\n <li>Implements, evaluates, and modifies plan of care</li>\n <li>Integrates research and evidence-based knowledge into clinical practice</li>\n <li>In addition to the primary responsibilities of the Registered Nurses (RN), the Clinical Nurse Coordinator (CNC) will have the following key responsibilities:</li>\n</ul>\n<p><strong>Direct Patient Care</strong></p>\n<ul>\n <li>Educating patients and families: specific to rehabilitation services</li>\n <li>Telephone and email symptom triage</li>\n <li>Engage in meaningful patient follow-up and care coordination</li>\n <li>ex: post-procedure, medication side effects</li>\n <li>New patient orientation to the service line</li>\n <li>Caregiver support</li>\n <li>Assist with in-clinic procedures:</li>\n <li>(set up, consent, time out, and patient positioning)</li>\n <li>Assessment of needs for additional supportive care resources</li>\n <li>Prescription refills and addressing medication questions</li>\n <li>Result reviews</li>\n</ul>\n<p><strong>Indirect Patient Care</strong></p>\n<ul>\n <li>Supportive care referrals</li>\n <li>FMLA paperwork, disability paperwork, Ferry passes</li>\n <li>Coordination of outside clinical care</li>\n <li>Ensuring continuity of care with outside providers</li>\n <li>Collaboration with the research team for patients on trial</li>\n <li>Place physiatry return orders and Supportive Care referral orders as directed by MD</li>\n <li>Coordinate responses to patient inquiries via MyChart and phone</li>\n <li>Coordinate lab tests and notification of patients</li>\n <li>Coordinate with Supportive Care and medical care teams as needed, across Fred Hutch sites</li>\n <li>Facilitate external referrals for ongoing rehabilitation needs</li>\n <li>Manage Epic Rehab Medicine Clinical Support InBasket inquiries from patients, staff, or other providers</li>\n</ul>\n<p><strong>Communication and Teamwork</strong></p>\n<ul>\n <li>Attend regular team meetings with the provider and nursing teams</li>\n <li>Coordination of care with ancillary services</li>\n <li>Delegation of tasks to appropriate team members</li>\n <li>Coordinating patient schedules with the clinic team</li>\n <li>Collaboration with infusion RN and other members of the care team to ensure patient needs are met</li>\n <li>Real-time management and escalation of issues in the clinic</li>\n</ul>\n<p><strong>Professional Development and Clinic Engagement</strong></p>\n<ul>\n <li>Document all nursing care activities accurately and in a timely fashion.</li>\n <li>Demonstrate knowledge and understanding of patient privacy rights.</li>\n <li>Active participation in continuing education, departmental projects, and committees.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Required:</strong></p>\n<ul>\n <li>Graduated from an accredited school of nursing</li>\n <li>Must have a current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); and must upgrade to an MSL with primary state of residence, WA, within 60 days of hire. This requirement may be waived under specific circumstances, subject to approval.</li>\n <li>Applicants that do not have previous oncology experience will be provided additional training and support</li>\n <li>Current BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross) and renewal required every two years</li>\n <li>Additional certification may be required dependent upon the department. </li>\n <li>Critical thinking: the ability to practice the cognitive skills of analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledge</li>\n <li>Ability to develop clinical judgment.</li>\n <li>Time Management skills: the ability to organize and manage time and tasks independently</li>\n <li>Ability to communicate effectively by looking and listening for cues, asking open-ended questions, exploring cues, using pauses, screening responses, and clarifying responses.</li>\n <li>Ability to effectively listen by using reflecting, acknowledging, summarizing, empathizing, and paraphrasing skills.</li>\n <li>Ability to effectively provide information by checking what information the person knows already, giving small amounts of information at a time, using clear terms and avoiding jargon, avoiding detail unless it is requested, checking for understanding, and pausing and waiting for a response.</li>\n</ul>\n<p><strong>Preferred:</strong></p>\n<ul>\n <li>BSN-prepared nurse</li>\n <li>OCN (Oncology Certified Nurse) or BMT RN</li>\n <li>2 years of oncology nursing experience </li>\n <li>Previous telephone triage experience </li>\n <li>Previous outpatient clinic experience </li>\n <li>Takes initiative</li>\n <li>Good communication skills</li>\n <li>Skilled at multitasking and organization</li>\n</ul>\n<p>The hourly pay range for this position is from $47.64 to $88.19, and pay offered will be based on experience and qualifications.  Registered Nurses are paid on a step-scale basis and are also eligible for shift differentials and other pay premiums such as advanced degree, preceptor, and certification.  </p>\n<p> </p>\n<p>Although Fred Hutch is not sponsoring most H-1B visas at this time, candidates who already hold an H-1B sponsored by another organization and are currently in the U.S. may be eligible for this position.</p>\n<p> </p>\n<p>This position may be eligible for relocation assistance.</p>\n<p>This position is 0.6 FTE, 24 hours/week.</p>\n<p> </p>\n<p>Fred Hutchinson Cancer Center offers employees a comprehensive benefits package designed to enhance health, well-being, and financial security.
