This role serves as the first point of contact for patients referred into behavioral health services. The Specialist is responsible for obtaining patient consent for participation in Collaborative Care, ensuring seamless handoff of referrals to Behavioral Health Care Managers, and supporting the care team by coordinating services that address patients’ behavioral and psychosocial needs.</span></p>\n<p><br></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt; font-weight: bold\">Key Responsibilities</span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Serve as the </span><span style=\"font-family: Arial, sans-serif; font-size: 10pt; font-weight: bold\">initial liaison</span><span style=\"font-family: Arial, sans-serif; font-size: 10pt\"> and point of contact for patients referred to behavioral health services.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Conduct informed consent discussions with patients, ensuring understanding of the Collaborative Care Model.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Collect and document patient consents in compliance with organizational and regulatory standards.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Facilitate warm handoffs and timely referrals to Behavioral Health Care Managers (BHCMs).</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Maintain accurate and thorough documentation in the electronic health record (EHR).</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Coordinate with medical providers, BHCMs, and psychiatric consultants to ensure continuity of care.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Monitor referral completion and follow up with patients as needed.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Provide patients with information, resources, and support to improve access to care.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Participate in care team meetings and communicate relevant updates to the behavioral health team.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Maintain compliance with HIPAA and ethical standards of professional practice.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-family: Arial, sans-serif; font-size: 12pt; font-weight: bold\">Qualifications</span></p>\n<ul>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Bachelor's Degree </span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Strong interpersonal and communication skills with the ability to engage patients sensitively, clearly and effectively.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Knowledge of behavioral health and medical systems of care.</span></li>\n<li><span style=\"font-family: Arial, sans-serif; font-size: 10pt\">Detail-oriented with strong organizational and documentation skills.</span></li>\n<li><span style=\"font-family: Arial, sans-serif;
