    "description": "<h2>Overview</h2>\n<p><strong>PURPOSE STATEMENT:</strong></p>\n<p> </p>\n<p>Meadowwood Hospital is looking for a compassionate and clinically skilled Social Worker III providing PRN services, delivering comprehensive psychosocial assessments, crisis intervention, and discharge planning for patients in acute behavioral health settings. Experienced in collaborating with interdisciplinary treatment teams to develop individualized care plans that promote patient stabilization, safety, and continuity of care. Adept at risk assessment, care coordination, community resource linkage, and supporting patients and families through complex mental health and substance use challenges. Committed to maintaining compliance with hospital policies, regulatory standards, and ethical social work practices while ensuring high-quality, patient-centered care. If this feels like an opportunity that you align with we look forward to your interest.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Conduct psychosocial assessment of patient and develop an effective treatment plan that applies therapeutic principles appropriately while ensuring patient safety at all times.</li>\n <li>Through patient observation and patient/family interview obtaining information on the patient’s needs, skills, interests and limitations for the development and implementation of the treatment plan.</li>\n <li>Provide crisis intervention and assist families in understanding the implications and complexities of a life situation and its impact on lifestyle. </li>\n <li>Participate in multidisciplinary treatment team meeting to discuss psychosocial assessment, treatment planning and ongoing discharge planning.</li>\n <li>Identify, assess and coordinate patient cases and anticipate their needs related to illness, hospitalization, finances, education and home/community services.</li>\n <li>Facilitate education, process groups, support groups and referrals as requested.</li>\n <li>Conduct intakes and assist in coordinating placements within facility programs as required.</li>\n <li>Develop and execute clinically relevant activities in coordination with activity professionals, as required.</li>\n <li>Promote recovery, symptom reduction and the achievement of the highest level of social functioning.</li>\n <li>Communicate or aid family members in understanding and supporting the patient.</li>\n <li>Manage a caseload and assume responsibility for patient chart documentation.</li>\n <li>Facilitate education, process groups, support groups and referrals as requested.</li>\n <li>Manage a caseload and maintain accurate and timely documentation to assure patient records comply with facility and accreditation standards.</li>\n <li>Supervise social work interns completing field placement, as required.</li>\n <li>Provide direction and guidance to social services staff or other staff members, as required.</li>\n <li>Demonstrate a positive, empathetic and professional attitude towards customers always.  When patient needs are not met, acknowledge and work to resolve complaints.
