All employees and potential employees will be recruited, selected, trained, promoted, compensated and, if necessary, disciplined or terminated without regard to sex, gender, race, ethnicity, national origin, citizenship, immigration status, religious affiliation, marital status, military veteran status, age, pregnancy, sexual orientation, gender expression or identity, disability, genetic information, or any other basis protected by law.</span></p>\n<p><br></p>\n<p><br></p>\n<p><span style=\"font-size: 14pt; font-weight: bold\"><span style=\"text-decoration: underline\">Job Summary</span></span></p>\n<p><br></p>\n<p>This position is responsible for providing supportive housing care coordinator services for households living in CCEW housing, discharging from Eastern State Hospital (ESH), or who qualify under the Medicaid Transformation Demonstration (MTD).  This position is also responsible for care coordination with partner agencies involved with households and assisting them in moving in a planned manner towards achieving personal independence.</p>\n<p><br></p>\n<p> </p>\n<p><br></p>\n<p><span style=\"font-size: 14pt; font-weight: bold\"><span style=\"text-decoration: underline\">Job Duties/Responsibilities</span></span></p>\n<p><br></p>\n<p>To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below.  Reasonable accommodation may be provided to enable individuals with disabilities to perform the essential duties/responsibilities of this job. </p>\n<p><br></p>\n<p> </p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"text-decoration: underline\">Supportive Housing: </span></span></p>\n<p><br></p>\n<ol>\n<li>For households discharging from Eastern State Hospital:\n<ol>\n<li>Support mechanisms that promote rapid and successful reintegration of consumers back into the community from psychiatric hospitals and long-term psychiatric placements, including participating with the hospital inpatient teams in treatment and discharge planning.</li>\n<li>Visit clients at ESH and Conduct intakes, develop client-centered individualized service plans, and complete ALTSA Housing Voucher packets within time limits specified by program requirements.</li>\n<li>Act as a care coordinator for all parties involved in the treatment team.</li>\n</ol>\n</li>\n<li>For households accessing Supportive Housing through MTD:\n<ol>\n<li>Where applicable, support chronically homeless households in applying for Supportive Housing services through Amerigroup.</li>\n</ol>\n</li>\n<li>Develop person-centered individualized integration plans and goals and interventions that support said plan.</li>\n<li>Assist households with finding housing in the community that meets their desired specifications.</li>\n</ol>\n<p><br></p>\n<p> </p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"text-decoration: underline\">Overall: </span></span></p>\n<p><br></p>\n<ol>\n<li>Provide Supportive Housing services according to professional ethical conduct guidelines.</li>\n<li>Act as landlord liaison during housing search. Facilitate background check screenings, application submissions, appeals when necessary, and inspection and lease signing appointments.</li>\n<li>Coordinate with landlords and treatment teams during clients’ tenancy to assure that tenancy issues are addressed pro-actively.</li>\n<li>In coordination with client’s care team, link recipients with primary care services and health homes; substance abuse treatment providers, vocational, education, employment, volunteer and social supports.</li>\n<li>Provide assistance in independent living skill-building, including financial and life skills coaching.</li>\n</ol>\n<p><br></p>\n<p><br><br></p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"text-decoration: underline\">Other duties/responsibilities: </span></span></p>\n<p><br></p>\n<ol>\n<li>Practice timely and complete timecard and file documentation management per program requirements.</li>\n<li>Provide information and referrals and link consumers with community resources as needed.</li>\n<li>Follow all COA standards of care for all clients.</li>\n<li>Provide outreach services as required.</li>\n<li>Uses appropriate administrative, fiscal, physical, and technical safeguards to ensure the confidentiality, integrity, and security of CCEW client protected health information (PHI), per regulations outlined in the Health Insurance Portability and Accountability Act of 1996 (HIPAA).</li>\n<li>Will regularly utilize all forms of communication (email, text, phone, etc.) and database practices necessary for this position and as directed by supervisor.</li>\n<li>As a mandated reporter, follows all procedures outlined in agency policies and procedures to report to the proper agency when there is suspicion and/or confirmation that a child or adult has been a victim of abuse or neglect.</li>\n<li>Other duties as assigned.</li>\n<li>Adheres to the tenets of Catholic Social Teaching and Catholic Doctrine.</li>\n<li>Perform as a team member to ensure that productivity outcome measures are achieved.</li>\n<li>Perform related functions necessary to support the mission and core values of Catholic Charities.</li>\n</ol>\n<p><br></p>\n<p><br><br></p>\n<p><span style=\"font-size: 14pt; font-weight: bold\"><span style=\"text-decoration: underline\">Job Qualifications</span></span></p>\n<p><br></p>\n<p>To perform this job successfully, an individual must meet the minimum qualifications listed below.  These qualifications are representative of the knowledge, skill and/or ability required to perform this job. </p>\n<p><br></p>\n<p> </p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><em>Education/Experience: </em></span><br>To perform this job successfully, an individual must have a BA in Psychology, or Social Work, or equivalent from an accredited academic institution.  Preference given to individuals with lived experiences with chemical dependency or mental health disorders.  Knowledge of community services and a minimum of two years’ experience in the direct treatment of mentally ill persons.  Experience working with issues of mental health and poverty required.</p>\n<p><br></p>\n<p> </p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><em>Certificates/Licenses:  </em></span>Must have a valid Driver’s License and ability to drive for work use.
