Avivo Village, one of our key programs, offers culturally responsive, low-barrier shelter and care to help residents achieve permanent housing stability.</p>\n\n<h3><strong>Your Role</strong></h3>\n\n<p>As a Site-Based Case Manager, you will support single adults with histories of chronic homelessness, many of whom live with substance use disorders. You will work closely with residents in our site-based program, Avivo Village. This role involves assertive engagement, harm reduction, and person-centered care to support residents in achieving housing stability and long-term goals.</p>\n\n<h3><strong>Key Responsibilities Include</strong></h3>\n\n<ul>\n\t<li>Build trusting relationships with clients through regular in-person engagement and creative outreach.</li>\n\t<li>Maintain a caseload and meet monthly with all clients; complete creative contact attempts.</li>\n\t<li>Conduct assessments and develop housing goal plans every six months.</li>\n\t<li>Support clients in maintaining housing, addressing landlord issues, and resolving crises.</li>\n\t<li>Provide eviction prevention and re-housing support, advocating for long-term stability.</li>\n\t<li>Complete timely and accurate documentation in required systems (HMIS, Credible, etc.).</li>\n\t<li>Collaborate with your team, assist with onboarding new staff, and attend required meetings.</li>\n\t<li>Participate in Avivo&rsquo;s Undoing Racism and cultural responsiveness initiatives.</li>\n</ul>\n\n<p><strong>Schedule: This is a 32/hr. a week full-time position.</strong></p>\n",
