This CHW position will focus primarily on assisting pregnant and post-partum women with an emphasis on decreasing infant mortality. The position will assist patients through a variety of methods, including clinic visits, phone visits, and home visits. CHW&#39;s will work closely with medical providers, staff, and other agencies to improve patient care and outcomes.&nbsp;<p><strong>Reports to</strong>: &nbsp;Women&rsquo;s Health Program Manager</p><p><strong>Manages</strong>: No</p><p><strong>Dress Requirement</strong>: Business Casual</p><p><strong>Work Schedule</strong>:</p><p>Monday through Friday during standard business hours</p><p>Times are subject to change due to business necessity</p><p><strong>Non-Exempt</strong>&nbsp;</p><p><strong>Requirements:&nbsp;</strong></p><p>&bull; Any combination of 3 years health/social services experience and/or education&nbsp;</p><p>&bull; Verifiable good driving record and reliable transportation&nbsp;</p><p>&bull; Background check and fingerprinting&nbsp;</p><p>&bull; <em>Bilingual (Spanish/Somali/Nepali) encouraged to apply&nbsp;</em></p><p><strong>Key Responsibilities:&nbsp;</strong></p><ul><li>Help to address patient social needs through phone visits, in person visits, and home visits. Help clients in utilizing resources, including scheduling appointments, and assisting with completion of applications for programs for which they may be eligible.</li><li>Follow-up with patients about health management/care plans with both patients and providers.
