This team member must be a dependable and compassionate Meal Delivery Driver to distribute nutritious food boxes to designated patients within the communities we serve. This role is a vital in ensuring timely and safe delivery of meals, contributing to the well-being of our community members. Responsibilities include:</p><ul><li>Safely pick up and deliver prepared food boxes to patient&rsquo;s residences following a predetermined route and schedule.</li><li>Conduct brief wellness checks during deliveries, noting any concerns and reporting them to the appropriate personnel.</li><li>Maintain accurate delivery records, including delivery confirmations.</li><li>Adhere to all traffic laws and company policies to guarantee safe transportation.</li><li>Connects individuals to health care services.</li><li>Provides basic health education/coaching to the community in order to prevent negative health outcomes.</li><li>Provides resource navigation assistance in order to meet individual&rsquo;s needs.</li><li>Facilitates and promotes access to services for vulnerable populations, including migrant farmworkers, the homeless, and other underprivileged communities</li></ul><p data-pasted=\"true\">What We&#39;re Looking For:</p><ul><li data-pasted=\"true\">Community Health Worker certification required. If not certified, must obtain certification within 1 year of employment.</li><li>Must possess a strong passion for community engagement work.</li><li>Lived experience or prior work experience with homeless populations is preferred.</li><li>Ability to communicate clearly and professionally in order to facilitate collaboration and cooperation both internally and externally.</li><li>Must be reliable and flexible in order to meet both the internal and external changing needs of the work.</li><li>Strong organizational skills and the ability to work autonomously.</li><li>Effectively manage assignments and projects.</li><li>Ability to prioritize and respond to multiple requests within requested time frames.</li><li>A valid driver&rsquo;s license is required.</li><li>Strong interest in working with an interdisciplinary, non-profit healthcare team as well as culturally and socioeconomically diverse community and patient population.</li><li>Proficiency with Microsoft Office programs and Electronic Medical Records.</li></ul><p data-pasted=\"true\">About Blue Ridge Health:</p><p data-pasted=\"true\">At Blue Ridge Health our mission is to improve Health, inspire Hope, and advance Healing through access to Compassionate, Affordable, and Quality Care.
