Offering assistance to the Elders as needed to get to their appointments. Follow the CPN CHR Transport policy. &nbsp;Assist the CHR nurse as needed.</p><p>CPN Health Services personnel may be assigned to provide services to patients of all ages including but not limited to Indian Children and will require a criminal history background screening.</p><p><br></p><p><strong>ESSENTIAL FUNCTIONS:</strong>&nbsp;</p><p>· Provide Tribal Elders safe transportation to and from medical appointments or other destinations as required.</p><p>· Give assistance as needed to get the Elder to their appointments--this may include helping them with their assistive devices or with any paperwork they are given at their appointment.</p><p>· Document all mileage, addresses, and client signatures as required on Transport Logs.&nbsp;</p><p>· Promptly report any problems encountered with the SSN Supervisor.</p><p>· Provide friendly and courteous customer service.</p><p>Performs assigned supportive procedures for the well-being of tribal elders.</p><ul><li>Provide patient advocacy and case and resource management.</li><li>Assist with personal care/Activities of Daily Living for patients: nutrition, shopping, running errands, and minor house cleaning,&nbsp;</li><li>Provide support to CHR nurse as needed.</li></ul><p><strong>ADDITIONAL DUTIES:</strong></p><ul><li>Perform other duties and projects as required by management and provides support as needed and defined by the CHR nurse.</li><li>Make home visits ordered by physician or SSN Project Director.</li><li>Participates in the community health promotion and disease prevention events.</li></ul><p><strong>KNOWLEDGE, SKILLS, AND ABILITIES:</strong></p><ul><li>Ability to communicate effectively and accurately with patients and staff.</li><li>Ability to maintain strict confidentiality regarding all patients encounters.</li><li>Computer skills</li><li>Specialized knowledge, skills, and abilities to successfully perform duties of the position and to recognize important status changes with the elder patient and report accordingly.</li></ul><p><strong>MINIMUM QUALIFICATIONS FOR CONSIDERATION:</strong></p><ul><li>Certified or willing to be certified as CHR upon request within one year of employment.</li><li>Certified in Basic Life Support (CPR) or becomes certified within 3 months of employment.</li><li>Must have a valid driver’s license and driving record that allows access to CPN vehicles for daily work function.</li><li>Computer skills required for documentation in the patient record.</li><li>Must have a High School diploma or GED equivalent</li><li>Must have a good driving record and personal automobile insurance</li><li>Must have very good communication and written skills</li><li>Ability to work a varying schedule as needed.</li></ul><p><strong>PHYSICAL REQUIREMENTS:</strong></p><ul><li>Sitting or standing or walking for long periods of time; occasional bending, squatting, kneeling, stooping; talking, hearing, and visual acuity</li><li>Frequent lifting (up to 50 lbs) and/or occasional lifting (up to 75 lbs)</li><li>Good driving skills</li></ul><p><strong>ENVIRONMENTAL CONDITIONS:</strong></p><ul><li>General clinical and home environment with frequent exposure to &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;chemicals, blood borne pathogens and patient body fluids.</li><li>Varying environments such as medical offices, hospitals, homes, &nbsp;&nbsp;&nbsp;&nbsp;&nbsp;and other environments; differing vehicles depending on transport needs.</li></ul><p><em><strong>Note:</strong></em> <em>The above statements are intended to describe the general nature and level of work performed by an employee in this position.</em> <em>These statements are not to be construed as an exhaustive list of all responsibilities, duties, and skills required of employees in this position.</em></p>",
