          "text": "<p>As a Registered Practical Nurse (RPN) leader of our frontline care team, the Resident Care Coordinator - Rehab Lead plays a vital role in the Home. Your voice, ideas, and leadership help shape daily care practices, while your presence and energy contribute directly to creating meaningful moments and smiles for our Residents who live here.</p><p>If you were working with us as a Resident Care Coordinator - Rehab Lead, here are some of the things you would have done last week:</p><ul><li>Led&#xa0;the Rehabilitation program using your knowledge and understanding&#xa0;of the principles of rehab programs – e.g. ambulation, mobility, communication, fall prevention, dining, ADLs, Active ROM</li><li>Provided clinical leadership in your assigned resident home area as a Registered Practical Nurse in good standing with the College of Nurses of Ontario</li><li>Led and supported the front-line PSW team by clearly communicating organizational goals, priorities, vision, and policies to achieve the best possible resident outcomes</li><li>Ensured Resident-Directed Care principles were consistently applied in daily care practices</li><li>Oversaw clinical programs and ensured compliance with Long-Term Care legislation, Ministry of Health requirements, Health &amp; Safety standards, and nursing standards of practice</li><li>Participated in Home-wide quality improvement activities, including data collection, analysis, reporting, and contributing to Quality Improvement Plans (QIPs) in alignment with HQO principles</li><li>Collaborated with the Leadership Team on educational needs within the Home and your assigned resident area</li><li>Supported, oriented, and onboarded newly hired PSW Team Members, including training on PointClickCare and Point of Care documentation practices</li><li>Coordinated and led PSW team meetings and daily home-area huddles to promote communication, accountability, and teamwork</li><li>Managed PSW performance-related matters, including coaching, performance appraisals, discipline, and participation in hiring or terminations as required</li><li>Acted as a point of contact for resident and family concerns, addressing issues proactively to prevent escalation</li><li>Advocated for Resident Rights and supported the development and ongoing review of individualized Resident Care Plans</li><li>Maintained accurate and up-to-date documentation related to residents’ conditions, progress, care instructions, and evaluations</li><li>Participated in Care Conferences and worked closely with a multidisciplinary team to support resident care planning</li><li>Kept current with best practices, emerging trends in gerontology, and nursing practice to continuously improve care delivery</li><li>Demonstrated positive, hands-on leadership by example while working collaboratively with the broader leadership team</li><li>Adjusted work hours as needed to support the operational needs of the Home, including evenings or weekends</li><li>Provided support to the MRC and Nursing Department as required and completed other duties as assigned by your supervisor</li></ul><p>When you apply, if you have your registration in good standing with the CNO, we will try our best to reach out to you within the week (hopefully within a few days).
