Case Manager MHP Victim Services
$1,000 Sign on Bonus
The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Case Manager Housing Locator & Mental Health Professional (MHP) Victim Servies who demonstrates this mission and wants to work for an organization that makes a difference in the community.
Schedule: Monday-Friday 9am-5m (24/7 on call rotation)
What will you be doing?
The Victim Services Case managers provide assertive outreach and case management, evidence-informed mental health and support services, and crisis intervention to victims/families in crisis or quality of life concerns. Case managers actively engage with individuals who have experienced trauma or have mental health concerns to provide both clinical intervention and case management, mental health interventions, resource linkage, and advocacy services while addressing their multifaceted social and tangible needs. Case managers participate in community events, participate in ride a longs and police department roll calls. Case Managers will be available to work necessary overnight and weekend shifts to continue to staff a viable Victim Services program and assist with crisis line coverage when requested. Case Managers will be available to work in-house when assigned to a police department and flexible to cover other police departments or assignments.
Essential Job Responsibilities:
Attend to scheduled work hours and be flexible to meet client and program needs, as assigned by Program Manager or Director.
Respond to outreach calls from shift start to shift end.
Respond to calls upon being dispatched
Complete assigned office hours.
Attend soup kitchens, breakfasts and community dinners
Complete all mandatory and additional trainings
Provide advocacy and support to individuals and families experiencing crime/trauma or quality of life concerns
Complete documentation on a daily basis, including Victim Service case log, daily log, and case notes.
Provide a minimum of three (3) follow-ups and three (3) appropriate referrals with assigned cases
Receive referrals from participating law enforcement agencies, assess the need for services, identify appropriate services, and conduct appropriate introductions to available services.
Manage case load and follow up with clients on a weekly basis or per clients request based on need.
Follow requirements on VCVA related cases
Assist law enforcement as requested
Provide educational trainings, presentations, and death notifications as requested.
Participate in field training (job shadowing) for 30 days or until approved to work independently on cases.
Acquire/maintain certification in First Aid, CPR, CPI, Mandated Reporting, ASIST training and 40 hour DV/SA training, and other agency required trainings.
Maintain and update Outlook calendar with daily details of employment activities.
Complete and maintain monthly and quarterly assigned reports.
Participate in ride along and roll calls at least once a month for all contracting agencies.
Complete a needs assessments for level of care.
Respond promptly (30-45 minutes based on location) to call outs
Respond promptly to referrals during regular work hours (30 minutes to and 1 hour based on location)
Stay within a 30 minute radius from communities served when primary on-call and 45- minute when second-on call
Maintain appropriate documentation for in-house case loads
Manage schedule in a manner that allows timely completion of all duties and be available to program after hours.

Qualifications
Qualifications:
Education: Bachelor's degree in human services field required Masters Degree preferred. Bilingual (Spanish) preferred.
License or Certification: ASSIST and 60 Hour DV/SA certification preferred.
Experience: Experience in crisis/emergency relief work, with at-risk, trauma and/or crisis clientele preferred.
Physical:
Position requires ability to communicate in-person, telephone and in group settings
Requires frequent standing; walking, n avigation of stairs on a regular basis, Ability to provide services in clients’ homes and in the community
No lifting restrictions
Equipment: Laptop, Copy Machine, FAX, Cell phone, Telephone with voice mail
Additional Requirements:
24-HourAvailability: flexible hours when necessary-evening, weekends, and holidays
May need to respond to all staff call out during major critical incidents overriding any prescheduled time off or other employment
Valid Illinois driver’s license
Drive self and clients in personal vehicle
Must have the minimal amount of liability insurance on personal automobile as defined by the AID handbook

We are looking for a Case Manager Housing Locator & Mental Health Professional (MHP) Victim Servies who demonstrates this mission and wants to work for an organization that makes a difference in the community. \\r\\n\\r\\nSchedule: Monday-Friday 9am-5m (24/7 on call rotation)\\r\\n\\r\\nWhat will you be doing?\\r\\n\\r\\nThe Victim Services Case managers provide assertive outreach and case management, evidence-informed mental health and support services, and crisis intervention to victims/families in crisis or quality of life concerns. Case managers actively engage with individuals who have experienced trauma or have mental health concerns to provide both clinical intervention and case management, mental health interventions, resource linkage, and advocacy services while addressing their multifaceted social and tangible needs. Case managers participate in community events, participate in ride a longs and police department roll calls. Case Managers will be available to work necessary overnight and weekend shifts to continue to staff a viable Victim Services program and assist with crisis line coverage when requested. Case Managers will be available to work in-house when assigned to a police department and flexible to cover other police departments or assignments. \\r\\n\\r\\nEssential Job Responsibilities:\\r\\n\\r\\n\\r\\n\\tAttend to scheduled work hours and be flexible to meet client and program needs, as assigned by Program Manager or Director. \\r\\n\\r\\n\\t\\r\\n\\t\\tRespond to outreach calls from shift start to shift end. \\r\\n\\t\\tRespond to calls upon being dispatched \\r\\n\\t\\tComplete assigned office hours. \\r\\n\\t\\tAttend soup kitchens, breakfasts and community dinners \\r\\n\\t\\tComplete all mandatory and additional trainings \\r\\n\\t\\r\\n\\t\\r\\n\\tProvide advocacy and support to individuals and families experiencing crime/trauma or quality of life concerns \\r\\n\\tComplete documentation on a daily basis, including Victim Service case log, daily log, and case notes.\\r\\n\\tProvide a minimum of three (3) follow-ups and three (3) appropriate referrals with assigned cases\\r\\n\\tReceive referrals from participating law enforcement agencies, assess the need for services, identify appropriate services, and conduct appropriate introductions to available services.\\r\\n\\tManage case load and follow up with clients on a weekly basis or per clients request based on need. \\r\\n\\tFollow requirements on VCVA related cases \\r\\n\\tAssist law enforcement as requested \\r\\n\\tProvide educational trainings, presentations, and death notifications as requested. \\r\\n\\tParticipate in field training (job shadowing) for 30 days or until approved to work independently on cases. \\r\\n\\tAcquire/maintain certification in First Aid, CPR, CPI, Mandated Reporting, ASIST training and 40 hour DV/SA training, and other agency required trainings. \\r\\n\\tMaintain and update Outlook calendar with daily details of employment activities. \\r\\n\\tComplete and maintain monthly and quarterly assigned reports. \\r\\n\\tParticipate in ride along and roll calls at least once a month for all contracting agencies. \\r\\n\\tComplete a needs assessments for level of care. \\r\\n\\tRespond promptly (30-45 minutes based on location) to call outs \\r\\n\\tRespond promptly to referrals during regular work hours (30 minutes to and 1 hour based on location) \\r\\n\\tStay within a 30 minute radius from communities served when primary on-call and 45- minute when second-on call \\r\\n\\tMaintain appropriate documentation for in-house case loads \\r\\n\\tManage schedule in a manner that allows timely completion of all duties and be available to program after hours.\\r\\n\\r\\n\\r\\nWhat will we provide Full Time employees.
