Under general supervision, to guide consumers to be self-directed by teaching consumers decision making skills and encouraging independence as defined in each person&#39;s Personal Support/Habilitation Plan. To provide assistance in a manner that supports the personal dignity of the consumers.</p><p><strong>______________________________________________________________________</strong></p><p><strong>&nbsp;</strong></p><p><strong>Essential Duties and Responsibilities</strong></p><p>&nbsp;</p><p>&nbsp;</p><p>Shall comply with all rules, regulations, Policies and Procedures of the Company</p><p>&nbsp;</p><p>Shall provide physical assistance to individuals as needed</p><p>&nbsp;</p><p>Shall provide residential training to facilitate the acquisition of communication skills, sensorimotor skills, and daily living and self-help skills</p><p>&nbsp;</p><p>Shall provide assistance with daily living, such as ambulation, dressing, grooming, eating, toileting, bathing, meal planning and preparation, laundry, budgeting and financial assistance, home care and cleaning</p><p>&nbsp;</p><p>Shall provide emotional support, advice and assistance to help the individual work through problems</p><p>&nbsp;</p><p>Shall communicate with the assigned Case Manager on a regularly scheduled basis and as needed</p><p>&nbsp;</p><p>Shall provide or make arrangements for transportation to services, activities and medical appointments as needed</p><p>&nbsp;</p><p>Shall actively participate in medical appointments and follow up care as directed by the medical staff</p><p>&nbsp;</p><p>Shall cooperate with monitoring to include periodic announced and unannounced visits to the home by various monitoring entities</p><p>&nbsp;</p><p>Shall assist individuals with the administration of their medications and ensure that medications are properly accounted for and properly labeled</p><p>&nbsp;</p><p>Shall administer all aspects of residence operation; including development and maintenance of a household budget</p><p>&nbsp;</p><p>Shall assist with menu planning and grocery shopping as needed</p><p>&nbsp;</p><p>Shall participate in household cleaning if necessary</p><p>&nbsp;</p><p>Provide direct support to consumers</p><p>&nbsp;</p><p>Assist consumers in performing daily living functions such as personal hygiene, dining skills, household chores and community integration, etc.</p><p>&nbsp;</p><p>Ensures all personal goals are worked on a daily basis and appropriate documentation is completed</p><p>&nbsp;</p><p>Administers medications at the appropriate time (S); notifies management and completes Medication Error form if an error occurs&nbsp;</p><p>&nbsp;</p><p>Assist with meal service; teach dining skills and promote social skills and personal dignity during meals</p><p>&nbsp;</p><p>Attend and successfully complete in-service training sessions as scheduled</p><p>&nbsp;</p><p>Ensures safe and clean living conditions at all times</p><p>&nbsp;</p><p>Complete data collection, observation notes and daily reports as instructed.</p><p>&nbsp;</p><p>Ensures that individual has choice of community activities and provides safe transportation&nbsp;</p><p>&nbsp;</p><p>Ensures / provides safe transport of consumer (in personal vehicle) to and from destination(S) as needed while on duty</p><p>&nbsp;</p><p>Assess the development of the consumer&#39;s abilities and interests to provide information for their Personal Support/Habilitation Plan.</p><p>&nbsp;</p><p>Participate in Interdisciplinary Team meetings to formulate the consumer&#39;s Personal Support/Habilitation Plans.</p><p>&nbsp;</p><p>Monitor consumer&#39;s activities to assure that safety and health care practices are observed.&nbsp;</p><p>&nbsp;</p><p>Ensure Universal precaution practices are observed.</p><p>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</p><p>Reports to the Field Coordinator all incidents</p><p>&nbsp;</p><p>Maintains confidentiality at all times and ensure HIPPA laws are followed</p><p>&nbsp;</p><p>Ensures all individuals retain cash receipt for all purchases; completes all appropriate financial documentation</p><p>&nbsp;</p><p>Monitors, addresses per individual&rsquo;s plan documents each individual&rsquo;s behaviors (S)</p><p>&nbsp;</p><p>Documents incident or injury on appropriate forms; notifies supervisor immediately in event of the injury&nbsp;</p><p>&nbsp;</p><p>Must maintain all compliance requirements outlined for the position of Direct Care Staff; Reliable insured transportation, &nbsp;
