Collaboration & Care Coordination Coordinate care with internal teams (case managers, peer support) and external partners (schools, primary care, courts). Participate in case consultations and interdisciplinary team meetings . Maintain strong communication with referral sources and stakeholders to ensure continuity of care. Documentation & Compliance Complete all clinical documentation including assessments, treatment plans, progress notes, safety plans, and discharge summaries . Ensure compliance with agency standards, MassHealth requirements, and regulatory guidelines . Participate in audits, reviews, and quality improvement initiatives. Professional Development Engage in weekly clinical supervision and ongoing training opportunities. Maintain or pursue clinical licensure as applicable. Contribute to agency initiatives focused on trauma-informed care, equity, and staff wellness . What You Bring Strong clinical skills in assessment, diagnosis, and therapeutic intervention Knowledge of trauma-informed care and evidence-based practices Ability to manage crisis situations and provide effective intervention Excellent communication, documentation, and organizational skills Commitment to culturally responsive, person-centered care Qualifications Master’s degree in Social Work, Psychology, Counseling, or related field License-eligible (LICSW, LMHC, LMFT, or Licensed Psychologist) CANS Certification Valid driver’s license (Class C or D)
