Sun River Health is a Federally Qualified, Non-Profit Health Center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County.&nbsp;</p>\n<p>We are currently seeking a full-time Psychiatric Nurse Practitioner to join our team and support multiple sites on Long Island.</p>\n<p><strong>Position Summary</strong></p>\n<p>A Psychiatric Nurse Practitioner provides psychiatric clinical services for patients. Performs job duties in compliance with systems and job specific standards.</p>\n<p><strong>Job Type:</strong> Full-Time</p>\n<p><strong>Work Schedule:</strong> Monday in Wyandanch, Tuesday in Patchogue, Wednesday and Thursday in Brentwood, and Friday in Coram. (This schedule can change.)</p>\n<p><strong>Salary Range:</strong> $159,724.50 - $172,614 per year&nbsp;</p>\n<p><strong>Sign-On Bonus:</strong> $3,500 Sign-On Bonus for Eligible External Applicants – Terms and Conditions Apply.</p>\n<p><strong>Essential Duties and Responsibilities</strong></p>\n<ul>\n <li>Consult with MD/Psychiatrist/LCSW on patients via eCW, telephone encounter, and/or with patient visits</li>\n <li>Consult with MD/Psychiatrist on medication management, via eCW, telephone encounter, and/or visit with patient as needed to address BH concerns</li>\n <li>Provide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals, in support of ongoing care by MD/Psychiatrist/LCSW, with referral back to ongoing care with clinical team</li>\n <li>Participate in case conferencing and high-risk meetings with LCSW/MD/Psychiatrist and care teams</li>\n <li>Approval of treatment plans for patients in BH care at Sun River</li>\n <li>Participate in efforts to support Sun River’s primary care and BH integration efforts</li>\n <li>Ensure appropriate transitions of care with external providers, PCP, hospitals, etc.</li>\n <li>Respond to Sun River clinical teams for crisis intervention</li>\n <li>Interviews, diagnoses and evaluates patients referred for psychiatric assessment</li>\n <li>Participates as a team member in conferences concerned with diagnosis and treatment planning</li>\n <li>Writes reports and documentation as required</li>\n <li>Consults with other mental health and medical provider staff regarding treatment of patients</li>\n <li>Prescribes psychotropic medication as needed and monitors its effectiveness</li>\n <li>Provides in-service training to professional and paraprofessional staff</li>\n <li>Reviews case records to monitor quality of care</li>\n <li>Must demonstrate knowledge and skill necessary to provide care appropriate to the age of the patients served</li>\n <li>Must demonstrate knowledge of the principles of growth and development over the life span</li>\n <li>Must be able to identify and categorize each patient’s age–specific grouping of needs, such as those for infant, adolescent, or geriatric patients</li>\n <li>Must be familiar with the process of diagnosing and treating patients with substance use disorder and make appropriate referrals when necessary</li>\n</ul>\n<p><strong>Essential Job Qualifications</strong></p>\n<ul>\n <li>Minimum Education Requirement: Master's Degree</li>\n <li>Minimum Licensure: Nurse Practitioner in Psychiatry and CPR/BLS</li>\n <li>Required Skills &amp; Knowledge:<br>\n   - Knowledge of diagnostic and treatment procedures that nurse practitioners are trained to do and are necessary for practicing in health care centers.<br>\n   - Competence in delivery of health care to patients (including preventative health education)<br>\n   - Ability to collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.<br>\n   - Bilingual in both English and Spanish (orally and written) is preferred</li>\n</ul>\n<p><strong>About Sun River Health</strong></p>\n<p>Sun River Health has a rich history of providing quality, affordable health care to communities in need.
