Join us in going above and beyond.</p><p>Position Summary: A Speech-Language Pathologist evaluates and treats patients per physician order utilizing a variety of therapeutic techniques. Speech-Language Pathologist communicates with families, physicians and other health team members regarding residents, maintains timely documentation in each medical record</p><p style='margin-top:0in;margin-right:0in;margin-bottom:0in;margin-left:0in;line-height:normal;font-size:15px;font-family:\"Calibri\",sans-serif;' id=\"isPasted\"><strong><span style=\"font-size:16px;\">Essential Duties:&nbsp;</span></strong></p><ul style=\"margin-left: 29px;\"><li style=\"font-size: 14px;\">Determines each patient&#39;s need for skilled therapy based on objective and documented criteria</li><li style=\"font-size: 14px;\">Evaluates patients within established protocol of physician&rsquo;s order</li><li style=\"font-size: 14px;\">&nbsp;Conducts the evaluation according to the standardized or recommend techniques and procedures including timely documentation</li></ul><div style='margin-top:0in;margin-right:0in;margin-bottom:10.0pt;margin-left:0in;line-height:115%;font-size:15px;font-family:\"Calibri\",sans-serif;'><ul style=\"margin-bottom: 0in; margin-left: 29px;\"><li style=\"margin: 0in 0in 10pt; line-height: 115%; font-size: 14px; font-family: Calibri, sans-serif;\">&nbsp;Develops treatment plan at the end of the evaluation which includes measurable and objective long- and short-term goals for each patient based on the evaluation data.</li></ul></div><ul style=\"margin-left: 29px;\"><li style=\"font-size: 14px;\">&nbsp;Follows each treatment plan in an objective, measurable and documented approach</li><li style=\"font-size: 14px;\">&nbsp;Requests assistance or adjusts therapy scheduled to avoid interference with the quality of service</li><li style=\"font-size: 14px;\">Maintains timely and accurate records according to policies, procedures and regulations with included evaluations, screenings, progress notes, discharge summaries, labor and service logs</li><li style=\"font-size: 14px;\">Provides written updates to physicians utilizing physician progress note pages in medical chart, &nbsp;in addition to Speech therapy section notes.</li><li style=\"font-size: 14px;\">Recommends Diet texture changes to physician on day of evaluation; follows individual Physician guideline to change texture.</li><li style=\"font-size: 14px;\"><span style=\"font-family: Calibri;\">Follows established protocols, including proper documentation for swallow precautions.&nbsp;</span></li><li style=\"font-size: 14px;\"><span style=\"font-family: Calibri;\">&nbsp;Maintains active and supportive communications with the patient, significant others, physician &nbsp;</span>and other team members concerning progress, problems, home programs, follow-up, and other issues related to the therapeutic process.
