    "requisitionDescription": "<div><p><strong>Job Purpose</strong></p><p>The UBHCC Nurse is responsible for supporting the multidisciplinary crisis team by delivering coordinated, high-quality care to clients at the Urgent Behavioral Health Crisis Center.&nbsp;</p><p><strong>Qualifications</strong>&nbsp;</p><ul style=\"list-style-type: disc;margin-left: -23.5px;\"><li>Must be a licensed LPN.</li><li>Experience in psychiatric or behavioral health care and healthcare technology is preferred.</li><li>Ability to conduct basic nursing assessments.&nbsp;</li></ul><p style=\"margin-left:-23px;\"><strong>&nbsp; &nbsp; &nbsp; &nbsp;Core Job Duties</strong></p><ul style=\"margin-left: -23.5px;\"><li>Obtain and record vital signs.</li><li>Perform blood and urine collection as requested by prescribers.</li><li>Reconcile home medications.</li><li>Complete triage assessments.</li><li>Based on triage assessment outcomes, determine appropriate care pathways and facilitate timely engagement with the next provider to ensure efficient service delivery.</li><li>Monitor clients&rsquo; physical and behavioral health status during their visit.</li><li>Communicate relevant information to the clinical team.</li><li>Conduct violence risk screening using the agency screening tool.</li><li>Complete nursing assessments.</li><li>Review information from PDMP (Prescription Drug Monitoring Program).</li><li>Provide medication education.</li><li>Facilitate telehealth connections for Nurse Practitioner evaluations.</li><li>Call in prescriptions and refill medications as directed by prescribers.</li><li>Complete documentation in a timely manner.</li><li>Communicate information orally in an effective and accurate manner.</li><li>Assure program operates with the constraints of the agency&rsquo;s certification (CARF, DMH) standards and strategic plan related to clinical care.&nbsp;</li><li>Abide by program and agency policies and procedures.</li><li>Other Duties: Participate in special projects and agency initiatives as assigned.</li></ul><p><br></p></div>\n",
