They provide care and help steer the interdisciplinary hospice team ensuring that our patients' physical, emotional and spiritual needs are met. RNCM's educate patients and families about the disease process, help manage and control symptoms, and offer support and strength during difficult times. Care delivery takes place in a community or where the patient resides. <br><br><strong>Responsibilities:</strong></p><ul><li>San Antonio hospice nursing care.</li><li>Foster therapeutic relationships.</li><li>Conduct thorough nursing assessments.</li><li>Plan and implement effective care.</li><li>Effectively manage pain and symptoms.</li><li>Ensure patient safety.</li><li>Educate patients, families, and caregivers on death and dying.</li><li>Support patients and families through the end-of-life processes.</li><li>Address end-of-life ethical concerns.</li><li>Produce accurate documentation in a timely fashion.</li><li>Maintain Bloom's professional conduct standards.</li></ul><strong>Qualifications:</strong><ul><li>Accredited nursing school graduate.</li><li>Valid TX RN license.</li><li>At least one year in med-surg, emergency or critical care nursing.</li><li>Strong clinical assessment capability.</li><li>Excellent communication and interpersonal skills.</li><li>Effective problem-solving skills.</li><li>Detail-oriented with strong time management.</li><li>Computer proficiency.</li></ul><strong>Additional Qualifications Preferred:</strong><ul><li>Experience in hospice or oncology.</li><li>Understanding of death/dying and bereavement processes.</li><li>CHPN certification.</li></ul><p><em>Bloom Healthcare is an equal opportunity employer.
