Description
We are seeking a compassionate and skilled Community Paramedic to support a grant-funded initiative focused on improving patient outcomes, reducing hospital readmissions, and enhancing access to preventative and community-based healthcare services.
This role provides non-emergency, patient-centered care in home and community settings, working closely with healthcare providers, social services, and community partners to address gaps in care and improve overall population health.
A Day in the Life of a Community Paramedic
You start your day reviewing your patient list and care plans, preparing to meet individuals where they are—both physically and in their healthcare journey. Your first visit takes you into a patient’s home, where you conduct a thorough in-home assessment, checking vital signs, reviewing medications, and addressing any immediate concerns. You may provide treatments or reinforce care plans established after a recent hospital discharge.
As the day progresses, you stay closely connected with the broader care team. Between visits, you collaborate with physicians, nurses, case managers, and social workers, ensuring each patient’s care plan is coordinated, up to date, and responsive to their evolving needs.
In the field, your role goes beyond clinical care. During visits, you often uncover barriers affecting patient health—like limited access to transportation, food insecurity, or unstable housing. You take time to identify these social determinants of health and connect patients with appropriate community resources that can make a meaningful difference in their well-being.
Education is a key part of your impact. You guide patients through chronic disease management, explaining how to manage conditions like diabetes or heart failure, emphasizing medication adherence, and offering preventative care strategies that empower them to stay healthier at home.
Throughout the day, you carefully monitor patient progress, documenting each interaction in detail. Your notes ensure continuity of care, meet clinical standards, and support the reporting requirements tied to the grant program.
Every interaction ties back to a larger mission—helping reduce unnecessary emergency room visits and hospital readmissions. Whether it’s catching early warning signs or providing timely intervention, your proactive approach helps patients avoid crises.
You also play a role behind the scenes, contributing to program success by collecting and reporting data that measures outcomes and demonstrates the effectiveness of the community paramedicine model.
Before wrapping up your day, you review documentation, ensure all care provided aligns with clinical protocols, safety standards, and grant compliance, and prepare for the next day—knowing your work is improving access, outcomes, and quality of life for the community you serve.
Qualifications
Licensure:
Current State of Nebraska Paramedic License Current State of Nebraska Community Paramedic License Current Valid Driver’s License with an acceptable motor vehicle record that is acceptable to Regional West Medical Center’s insurance carrier Strong assessment, clinical, and critical thinking skills Ability to work independently in community-based settings Excellent communication and interpersonal skills Certifications/Credentials:
Current National Registry of Emergency Medical Technicians Paramedic Certificate International Board of Specialty Certification Community Paramedic Certification (CP-C) American Heart Association BLS, ACLS, PALS provider required, NRP provider preferred. NIMS IS 700 and 800 preferred NIMS IS100, IS200 required Skills & Competencies
1 year experience at the paramedic level with an Advanced Life Support Service with a total call volume of 1500 or more calls per year. Or 2 years or more experience at the paramedic level with an Advanced Life Support Service. Strong attention to detail and accuracy Ability to manage multiple tasks and interruptions in a fast-paced environment Effective communication and interpersonal skills Basic computer proficiency and ability to learn electronic health record systems Ability to maintain confidentiality and handle sensitive information Commitment to quality, safety, and patient-centered care
