JOB DESCRIPTION: The Social Worker will actively participate in the delivery of care and ongoing program development for the KidFit Health and Wellness Clinic. The KidFit Health and Wellness Clinic is a 2-year, family-centered, paediatric weight management & wellness program. The position involves working with the inter-professional KidFit team, conducting psychosocial assessments, and providing mental health support to help children and their families lead active and healthy lives. The patient population comprises a diverse mix of children and adolescents and their families with obesity. The role will support occasional coverage within the paediatric ambulatory care program. RESPONSIBILITIES: Practice from an integrated model that considers the implications of physical and mental health, the family’s cultural context and issues of social complexity Demonstrated skills in patient and family engagement, interviewing skills with individual, parents and families Identify strengths and risks within families that should be considered in the context of the assessment formulation and treatment recommendations Develop and facilitate psychoeducation/clinical groups for both individual and family/caregiver. Collaborate with a diverse inter-professional health care team Integrate research into practice; introduce innovation in clinical care to advance the program for youth and families Navigate and secure resources that address issues related to the social determinants of health Work collaboratively with other agencies and services such as CAS, Ontario Works, Children’s Mental Health agencies Support training and education through the supervision of MSW students Provide clinical teaching, mentoring and/or supervision for Social Work students. Engage with inter-professional education experiences with students of all Social Work health disciplines. QUALIFICATIONS: Master’s degree in Social Work (required) Registration with the Ontario College of Social Workers and Social Service Workers (required) Three to five years direct clinical experience working with children and families either in a community or hospital setting. (Required) Experience with disordered eating is considered an asset. Demonstrated skills in working with families experiencing crisis, as well as group facilitation skills in the delivery of psychoeducation for both induvial and caregivers. Knowledge and experience with crisis prevention and intervention models Familiarity and experience with mental health intervention models such as CBT, DBT, Narrative Therapy, Family Based Therapy is an asset Demonstrated experience and knowledge in culturally competent practice, and extensive understanding of the importance of health equity, diversity, and inclusion in the delivery of healthcare services Experience providing care using an anti-oppressive lens and critical reflexivity skills Demonstrated knowledge of, and sensitivity to, the impact of social, economic and political issues facing patients/families from diverse communities Experience collaborating with sectors inclusive of child protection, education, children mental health sectors; knowledge of the relevant network of community services and resources is an asset Capacity to collaborate flexibly and effectively as a member of the inter-professional health care team and to take initiative and leadership when appropriate Proficiency in the use of electronic platforms such as Microsoft Office, hospital documentation systems and other relevant technology Excellent interpersonal, written and oral communication, organizational, time management skills and attention to detail Demonstrated critical thinking and problem-solving skills Commitment and comfort working in a culture dedicated to quality improvement, evidence-based practice, clinician accountability and measurement-based care.
