The KidFit Health and Wellness Clinic is a 2-year, family-centered, paediatric weight management &amp; wellness program. The position involves working with the inter-professional KidFit team, conducting psychosocial assessments, and providing mental health support to help children and their families lead active and healthy lives.&nbsp; The patient population comprises a diverse mix of children and adolescents and their families with obesity.&nbsp; The role will support occasional coverage within the paediatric ambulatory care program.</p><p><strong><u>RESPONSIBILITIES:</u></strong></p><ul style=\"list-style-type: disc;\"><li>Practice from an integrated model that considers the implications of physical and mental health, the family’s cultural context and issues of social complexity</li><li>Demonstrated skills in patient and family engagement, interviewing skills with individual, parents and families</li><li>Identify strengths and risks within families that should be considered in the context of the assessment formulation and treatment recommendations</li><li>Develop and facilitate psychoeducation/clinical groups for both individual and family/caregiver.</li><li>Collaborate with a diverse inter-professional health care team</li><li>Integrate research into practice; introduce innovation in clinical care to advance the program for youth and families</li><li>Navigate and secure resources that address issues related to the social determinants of health</li><li>Work collaboratively with other agencies and services such as CAS, Ontario Works, Children’s Mental Health agencies</li><li>Support training and education through the supervision of MSW students&nbsp;</li><li>&nbsp;Provide clinical teaching, mentoring and/or supervision for Social Work students. Engage with inter-professional education experiences with students of all Social Work health disciplines.</li></ul><p><strong><u>QUALIFICATIONS:&nbsp;</u></strong></p><ul style=\"list-style-type: disc;\"><li>Master’s degree in Social Work (required)</li><li>Registration with the Ontario College of Social Workers and Social Service Workers (required)</li><li>Three to five years direct clinical experience working with children and families either in a community or hospital setting.
