Our HomeCare and Hospice services are part of Covenant Health, East Tennessee’s largest nonprofit health system and a Becker’s “Top 150 Places to Work in Healthcare.” Covenant Health includes nine hospitals and nearly 150 service locations, offering employees a comprehensive benefits package with tuition reimbursement, student loan assistance, certification bonuses, and leadership development programs. </p>\n<p> </p>\n<p><strong>Position Summary:</strong></p>\n<p>Provides professional social work services to designated patient population, and provides counseling and discharge planning as well as other social work services to patients and families. The general practice of social work in a home health setting includes interviewing, assessment, planning, intervention, evaluation, case management, mediation, supportive counseling, information and referral, problem solving, client education, and advocacy. This position has documented advance practice skills via certification and experience.</p>\n<h2>Responsibilities</h2>\n<ul>\n <li> Assists patients/family/significant other with physical, emotional, and social concerns that arise during the episode of care.</li>\n <li> Provides discharge planning services to patients needing this service.</li>\n <li> Completes required documentation prerequisites for placement/authorizations prior to discharge.</li>\n <li> Completes initial assessment and continued documentation to obtain pertinent background/social/ financial data. Documents social determinants of health as indicated in medical record.</li>\n <li> Connects patients/family/significant other with community resources to aid in their treatment and recovery.</li>\n <li> Acts as facility liaison with adult and children protective services as indicated.</li>\n <li> Collaborates with interdisciplinary team including physicians and keeps them informed of progress with patients via verbal communication and chart documentation in medical record.</li>\n <li> Exhibits the skills of empathy, communication, organization, critical thinking, active listening, and patience.</li>\n <li> Local travel required.</li>\n <li> Follows policies, procedures, and safety standards. Completes required education assignments annually. Works toward achieving goals and objectives, and participates in quality improvement initiatives as requested.</li>\n <li> Performs other duties as assigned.</li>\n</ul>\n<h2>Qualifications</h2>\n<p><strong>Minimum Education:</strong>            </p>\n<p>Bachelor’s degree from accredited social work program <u>with</u> state licensure (LBSW) <strong>OR</strong> Master’s degree (MSW) from accredited graduate social work program.</p>\n<p> </p>\n<p><strong>Minimum Experience:</strong>         </p>\n<p> Two (2) years of experience.</p>\n<p> </p>\n<p><strong>Licensure Requirement:</strong>     </p>\n<p>If Bachelor prepared, <u>must</u> be licensed (LBSW). No licensure required with MSW. Employee must have a valid Tennessee driver’s license and state mandated minimum insurance coverage. Driving record must meet Covenant Health minimum standards at the date of hire and throughout employment tenure.</p>",
