    "description": "<p><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">At Children’s Crisis Treatment Center (CCTC), our staff is dedicated to addressing the impact of childhood traumatic events and other challenges.  The Care Coordinator will support our Tsmss program and follow through with treatment team recommendations in a faced paced environment and meet the needs of the children ages 2-14 and their families presenting with crisis and behavioral health issues in a trauma-informed manner.</span></span></p>\n<p><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Summary of Essential Functions;</span></span></p>\n<ul>\n\t<li><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Identify resources within the targeted community ( e. g., MH/MR Centers, schools, day care centers, District Health Centers, recreational programs, legal assistance programs, housing assistance programs), as well as other relevant resources located throughout Philadelphia, establish relationships with these resources, and keep up-to-date records about these resources</span></span></li>\n\t<li><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Establish and maintain a positive working relationship with West-African refugee community, cultural, and religious groups/leaders.</span></span></li>\n\t<li><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Assess the service needs of refugee children and families and develop appropriate individualized service plans in collaboration with the children and families</span></span></li>\n\t<li><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Link refugee children and families to needed resources</span></span></li>\n\t<li><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Work w/ families to remove any barriers that may prevent participation in needed services (e. g., insurance)</span></span></li>\n\t<li><span style=\"font-size:12px;\"><span style=\"font-family:Arial, Helvetica, sans-serif;\">Work w/ refugee children and families on a regular basis (at least monthly face-to-face meetings) to assess progress and need for additional services/support.
