As part of our <strong>Certified Community Behavioral Health Clinic (CCBHC)</strong> team, you’ll help individuals and families access the care, resources, and support they need to thrive.</p><p><br></p><h3><strong>About the Role:</strong></h3><p>We’re looking for a <strong>Per Diem</strong> (on call) <strong>Care Coordinator</strong> to join our team in the Caribou Office.&nbsp;</p><p>In this role, you’ll be the connector—helping clients move through the behavioral health system, find critical services, and build a healthier future.</p><p><br><strong>Key Responsibilities:</strong></p><ul><li>Serve as a care coordinator and concierge for individuals enrolled in AMHC’s CCBHC program. &nbsp;</li><li>Complete triage assessments, support service access, and monitor referrals through systems such as Open Beds. &nbsp;</li><li>Facilitate support groups and social activities to build community connections. &nbsp;</li><li>Conduct triage interviews via telehealth across the organization. &nbsp;</li><li>Participate in discharge planning for individuals transitioning from institutional or higher levels of care to community-based services. &nbsp;</li><li>Conduct emergency evaluations for individuals in crisis. &nbsp;</li><li>Provide outreach, advocacy, and assistance with navigating MH/SUD treatment and healthcare benefits. &nbsp;</li><li>Participate in treatment and safety planning, providing consistent follow-up and care coordination. &nbsp;</li><li>Organize outreach efforts and support health and wellness initiatives in the community. &nbsp;</li><li>Maintain ongoing communication with external behavioral health providers and internal teams. &nbsp;</li><li>Identify and help remove barriers to care while promoting awareness of available resources. &nbsp;</li><li>Foster a culture of respect and empowerment, encouraging recovery-oriented approaches. &nbsp;</li><li>Document all services in the Electronic Health Record (EHR) and complete National Outcome Measures (NOMS) for CCBHC clients. &nbsp;</li><li>Ensure compliance with CCBHC guidelines, including culturally competent care and alignment with federal clinical standards. &nbsp;</li><li>Attend required meetings, uphold quality standards and ethical practices, and maintain professionalism in all interactions. &nbsp;</li><li>Occasionally, provide transportation for clients to ensure access to care.&nbsp;</li></ul><p><strong>What We Offer:</strong></p><ul><li>Competitive salary and benefits &nbsp;</li><li>Ongoing training and clinical supervision &nbsp;</li><li>Supportive, mission-driven work environment &nbsp;</li><li>Opportunity to make a real difference in rural Maine &nbsp;</li></ul><p><br></p><p><strong>Apply Today</strong> Join us in providing hope, stability, and connection when it’s needed most.<br><br><br></p><align-center><strong>AMHC is an Equal Opportunity Employer&nbsp;</strong></align-center>",
