This position is permanent, full-time, and is exempt.</span></p><p style='margin-top:0in;margin-right:0in;margin-bottom:3.0pt;margin-left:0in;line-height:normal;font-size:15px;font-family:\"Calibri\",sans-serif;color:#44546A;'><span style=\"font-family: tahoma, sans-serif; font-size: 14px;\"><br></span></p><h2 style='margin-top:11.0pt;margin-right:0in;margin-bottom:4.0pt;margin-left:0in;line-height:normal;font-size:17px;font-family:\"Calibri Light\",sans-serif;color:#44546A;font-style:italic;' id=\"isPasted\"><span style=\"font-size: 14px; font-family: tahoma, sans-serif; color: black;\">Duties/Responsibilities:</span></h2><ul><li id=\"isPasted\">Performs integrated behavioral health and substance use assessments and diagnosis per DSM-V diagnostic criteria; and makes behavioral health treatment recommendations using ASAM patient placement criteria.</li><li>Develops individualized treatment plans, updates and discharge plans consistent with specific problems identified in the assessment.</li><li>Provides appropriate treatment plan reviews, arranges for transitions to least restrictive services, and coordinates referrals as necessary.</li><li>Maintains clinical documentation in the agency&#39;s electronic health record that meets agency policies and procedures, accreditation requirements, and professional/ethical regulations.
