The LPN will report directly to the Director of Nursing. </span></p>\n<p><br></p>\n<p><br><span style=\"font-weight: bold\"><span style=\"font-size: 16px\">Job Responsibilities</span></span><span style=\"font-size: 16px\">:</span></p>\n<ul>\n<li>Explaining services by performing community outreach. He or she will provide education to the clients, hospital staff, physicians, community stakeholders on Home Care Services and the admission process. </li>\n<li><span style=\"font-size: 16px\">Promotes and restores patients' health by completing the nursing process; collaborating with physicians and multidisciplinary team members; providing physical and psychological support to patients, friends, and families; supervising assigned team members.</span></li>\n<li><span style=\"font-size: 16px\">Confer with the RN supervisor regarding needed changes in the Plan of Treatment/Care.</span></li>\n<li><span style=\"font-size: 16px\">Provide nursing assessments and patient education. </span></li>\n<li><span style=\"font-size: 16px\">Treat wounds, change wound dressings, giving injections, administer enema , and diabetic foot checks</span></li>\n<li><span style=\"font-size: 16px\">Collect specimens from patients </span></li>\n<li><span style=\"font-size: 16px\">Administers medications and set up medication dispensers.</span></li>\n<li><span style=\"font-size: 16px\">Measures and records vital signs</span></li>\n</ul>\n<p><br></p>",
