Submit your application now!</p><h2 id=\"isPasted\"><span style=\"font-size: 14px;\">POSITION SUMMARY</span></h2><p><span style=\"font-size: 14px;\">This position is responsible for providing case management and support services for individuals with substance use and mental health disorders to ensure treatment access and retention.&nbsp;</span></p><p><span style=\"font-size: 14px;\"><strong>ESSENTIAL DUTIES AND RESPONSIBILITIES&nbsp;</strong></span></p><ul><li style=\"font-size: 14px;\">Provide linkage to mental health and substance use treatment services, including a wide range of treatment modalities and recovery communities.</li><li style=\"font-size: 14px;\">Identify needs and assist individual in gaining access to financial benefits and other supportive services including but not limited to medical, social, housing, family planning and reunification, and educational services.</li><li style=\"font-size: 14px;\">Provide education and coaching, particularly in available resources within the local recovery community&nbsp;</li><li style=\"font-size: 14px;\">Monitor and track client progress</li><li style=\"font-size: 14px;\">Perform outreach activities to reach populations in need of services with emphasis on early intervention</li><li style=\"font-size: 14px;\">Assist individual in achieving goals established in recovery plan</li><li style=\"font-size: 14px;\">Assist with obtaining reliable means of transportation, including use of public transportation&nbsp;</li></ul><h2><span style=\"font-size: 14px;\">MINIMUM REQUIREMENTS</span></h2><ul><li style=\"font-size: 14px;\">High School diploma or GED equivalent</li><li style=\"font-size: 14px;\">Minimum of 2 years&rsquo; experience in related field</li><li style=\"font-size: 14px;\">Valid driver&rsquo;s license</li><li style=\"font-size: 14px;\">Prior knowledge of mental health and substance use strongly preferred</li><li style=\"font-size: 14px;\">Lived experience with mental health and/or substance use disorders strongly preferred</li><li style=\"font-size: 14px;\">Ability to learn new systems and programs quickly</li><li style=\"font-size: 14px;\">Excellent verbal and written communication skills</li><li style=\"font-size: 14px;\">Excellent customer service skills</li><li style=\"font-size: 14px;\">Ability to develop clear and professional documentation</li><li style=\"font-size: 14px;\">Willingness and ability to establish and maintain healthy working relationships&nbsp;</li></ul><p id=\"isPasted\"><strong>BENEFITS:</strong></p><ul type=\"disc\"><li>Competitive salary.</li><li>Schedule flexibility.</li><li>11 holidays, accrued pool of PTO starting at 25 days per year.</li><li>No on-call requirements.</li><li>In-house clinical supervision for LPC, LCSW, and LMFT.</li><li>National Health Services Corp (NHSC) Loan Repayment Program (LRP) at select locations.</li><li>Public Service Loan Forgiveness (PSLF) qualified agency.</li><li>Billing, marketing, and case management support.</li><li>Comprehensive medical, dental, and vision insurance.</li><li>Complimentary life Insurance, AD&amp;D, and Short-Term Disability.</li><li>403(b) retirement Plan with company match.</li></ul></div>\n    </div>\n  \n",
