We transform behavioral health systems through innovation, creativity and collaboration.</p>\n<p><br></p>\n<p><span style=\"font-weight: bold\"><span style=\"font-size: 18px\">Job Description:</span></span></p>\n<p><span style=\"font-size: 15px\">Responsible for providing case management services to participants within the program and will act as an advocate for the participant providing resources, case management, transportation as well as working with participants on pre- and post-treatment planning.</span></p>\n<p><span style=\"font-size: 16px\"> </span></p>\n<p><span style=\"font-weight: bold\"><span style=\"font-size: 18px\">R<span style=\"font-weight: bold\">OLES AND RESPONSIBILITIES:</span></span></span></p>\n<ul>\n<li><span style=\"font-size: 15px\">Maintains a relationship with all partnering agencies and referral sources.</span></li>\n<li><span style=\"font-size: 15px\">Efficiently document all billable interactions into AKAIMS, utilizing DAP note formatting.</span></li>\n<li><span style=\"font-size: 15px\"> Coordinates with referral sources, involved family members, community agencies and other treatment providers to provide the best treatment and case management options for the individual participant’s needs.</span></li>\n<li><span style=\"font-size: 15px\"> Provides motivational interviewing, social, and emotional support to participants entering and engaging in program.</span></li>\n<li><span style=\"font-size: 15px\"> Create crisis stabilization plans as needed for participants exploring treatment and coordinate assistance from peer supports to accomplish immediate/crisis goals.</span></li>\n<li><span style=\"font-size: 15px\"> Assists participants in identifying case management services; as needed, in accordance to best practices to provide linkage and advocacy that participants need. To include coordinating referrals that pertain to employment, mental health, housing, medical with local community clinics, social service agencies, continuing care and provide follow-up on all referrals made.</span></li>\n<li><span style=\"font-size: 15px\"> Works with participants to develop and update personalized plans prior to program completion to ensure participants leave the program with a clear plan for continued housing, employment, support, and treatment needs.</span></li>\n<li><span style=\"font-size: 15px\"> Assist participants in exploring available grants to aid them in getting funding for housing, transportation, education, and other needed items.</span></li>\n<li><span style=\"font-size: 15px\"> Attends all relevant trainings and direct support meetings.</span></li>\n<li><span style=\"font-size: 15px\"> Participate in training in areas related to improvement of service delivery.</span></li>\n<li><span style=\"font-size: 15px\"> Attends staff meetings and works collaboratively with other staff in case staffing/review.</span></li>\n<li><span style=\"font-size: 15px\"> Transport participants on assigned activities, case management services and outings as needed.</span></li>\n<li><span style=\"font-size: 15px\"> Maintains a respectful relationship with participants, former participants, family members and peers.</span></li>\n<li><span style=\"font-size: 15px\"> Performs other duties as needed and/or assigned.</span></li>\n</ul>\n<p><br></p>\n<p><span style=\"font-size: 18px; font-weight: bold\">QUALIFICATIONS AND EDUCATION REQUIREMENTS:</span></p>\n<ul>\n<li><span style=\"font-size: 15px\">Associate's degree in human services or related field.
