As a leader, you will embody these values and shape our culture.</p></li>\n <li><p><strong>Comprehensive Benefits:</strong> Enjoy a competitive benefits package, including medical/dental/vision coverage, an excellent 401k plan, tuition reimbursement, vacation, holiday, and sick time, long and short-term disability, and more.</p></li>\n <li><p><strong>Impactful Role:</strong> You will be responsible for coordinating the restorative nursing program, facilitating daily care coordination meetings, and ensuring timely communication with patients and families regarding changes in condition and discharge planning.</p></li>\n</ul>\n<p>#ND123</p>\n<h2>Responsibilities</h2>\n<ul>\n <li>Complete MDS Assessments</li>\n <li><p>Facilitate daily care coordination meetings to align care resources with patient needs and benefits.</p></li>\n <li><p>Participate in and facilitate Engage/72 HR Admission meetings as part of the care management system.</p></li>\n <li><p>Manage the restorative nursing program, integrating it with the activity department.</p></li>\n <li><p>Ensure timely communication with patients and families regarding changes in condition and discharge planning.</p></li>\n <li><p>Collaborate with the clinical team to develop and execute care plans.</p></li>\n <li><p>Coordinate discharge planning and post-discharge communication, including home health and DME.</p></li>\n <li><p>Monitor Quality Measures alongside the care coordination team.</p></li>\n <li><p>Serve as a skilled care documentation specialist, ensuring accurate and timely completion of Medicare and Managed Care regulatory guidelines.</p></li>\n <li><p>Review Additional Documentation Requests (ADRs) to ensure all documents meet the request.</p></li>\n</ul>\n<h2>Qualifications</h2>\n<li>Degree in nursing, licensed as Registered Nurse</li>\n<li>Experience in MDS</li>\n<li>Knowledge of healthcare reimbursement, utilization management, and discharge planning.</li>\n<li>Experience providing strategic leadership, managing teams and leading projects required.</li>\n<li>Ability to formulate and implement short and long term strategic plans</li>\n<li>Strong verbal, written and interpersonal skills.</li>\n<li>Must have proven ability to lead a team to achieve desired outcomes of team</li>\n<li>Strong communication and organizational skills required</li>\n<li>Must have knowledge of PDPM requirements of insurers</li>\n<li>Must have understanding of regulatory requirements</li>\n<li>Previous case management experience preferred</li>\n<p>(EOE)</p>",
