This position is eligible for a generous benefit package including health insurance, dental, vision, dependent care, paid time off including 4 weeks of vacation starting in the first year, 401(k), tuition assistance program and more. \\r\\n\\r\\n&nbsp;\\r\\n\\r\\nEssential Duties:\\r\\n\\r\\n\\r\\n\\t\\r\\n\\tAssign cases to case managers and schedules work according to priority;\\r\\n\\t\\r\\n\\t\\r\\n\\tMaintain necessary records and supervision notes; prepare regular and special reports as required;\\r\\n\\t\\r\\n\\t\\r\\n\\tOversee quality assurance of case management services to ensure compliance with Comunilife, and government policies and regulations;\\r\\n\\t\\r\\n\\t\\r\\n\\tPerforms audits of client charts to ensure comprehensive services planning and accurate charting of client progress;\\r\\n\\t\\r\\n\\t\\r\\n\\tEnsures that the home visit policy and procedures are enforced, tracked and documented.\\r\\n\\t\\r\\n\\t\\r\\n\\tProvide training to new and current case managers;\\r\\n\\t\\r\\n\\t\\r\\n\\tSpeak to dissatisfied clients and handle difficult situations;\\r\\n\\t\\r\\n\\t\\r\\n\\tReceive regular supervision with the&nbsp; Program Director (PD) and inform of the general status of cases, staff performance, problems and trends;\\r\\n\\t\\r\\n\\t\\r\\n\\tComplete probationary and annual performance reviews for case management staff;\\r\\n\\t\\r\\n\\t\\r\\n\\tDevelop and maintain cooperative relationships with ancillary agencies;\\r\\n\\t\\r\\n\\t\\r\\n\\tPrepare and submit all required program statistics and reports accurately and in a timely fashion;\\r\\n\\t\\r\\n\\t\\r\\n\\tUnder the supervision of the&nbsp; Program Director, develop comprehensive group programming, in compliance with contract and agency requirements;\\r\\n\\t\\r\\n\\t\\r\\n\\tCoordinate and supervise efforts with case management and maintenance staff to ensure a minimum monthly occupancy rate of 95% is maintained;\\r\\n\\t\\r\\n\\t\\r\\n\\tCoordinate and supervise efforts to place new OMH clients in vacated units within a maximum of 30 days from referral;\\r\\n\\t\\r\\n\\t\\r\\n\\tEnsure that the initial and medical assessments are conducted, and that comprehensive service plans are developed that identify mutually agreed upon long and short-term goals;\\r\\n\\t\\r\\n\\t\\r\\n\\tReview and provide supervision in the development of case management service plans and ensure that reassessments are conducted on a quarterly basis (at minimum) to ensure that services are appropriate for the client&rsquo;s current needs;\\r\\n\\t\\r\\n\\t\\r\\n\\tProvide oversight and supervision in the delivery of mental health and alcohol/substance abuse services and referrals, including assistance in accessing primary medical care, assistance with adherence to medication regimens, activities for daily living, nutritional services, recreation programs, assistance in employment assessments, training, private sector placement, prevention with positives, and delivery of any other services necessary for clients to increase their capacity for independent living;\\r\\n\\t\\r\\n\\t\\r\\n\\tEnsure that all client linkage agreements are monitored and updated, as necessary;\\r\\n\\t\\r\\n\\t\\r\\n\\tSupervise and coordinate efforts to provide assistance in obtaining and maintaining all government benefits and services for which the client is potentially eligible.
