Description

The licensed mental health provider at ACG Health is responsible for promoting and restoring patients’ health through trauma-informed, patient-centered psychotherapy. The incumbent is expected to meet a weekly productivity level of 25 clinical sessions, inclusive of individual and group therapy services. They will provide care primarily to patients living with HIV, and other comorbidities and to members of the LGBTQ+ community, as assigned and in accordance with departmental needs. We operate on a 35 hour work week and offer an excellent benefit package: 100% employer paid medical insurance PTO starting with your first full calendar year; PTO is prorated at time of hire 9 company holidays 401k Plan with 100% match on the first 3%, 50% match on additional 2%, vested immediately 100% Paid Parental and Medical leave SEPTA Benefits The anticipated annual salary range for this position is $62,000–$72,000. Compensation is determined based on experience and qualifications. In certain circumstances, offers may fall outside of the listed range. Responsibilities: Greet and acknowledge all patients in accordance with agency standards of professionalism, respect, and culturally responsive care Provide trauma-informed, patient-centered mental health services to individuals living with HIV and co-occurring medical and mental health conditions Conduct mental health screenings, biopsychosocial assessments, and treatment planning within scope of practice and agency standards Provide individual, couples, family, and/or group psychotherapy utilizing evidence-based psychotherapeutic approaches appropriate to patient needs Collaborate with medical providers and interdisciplinary team members to coordinate integrated patient care and support continuity of services Participate in regular case consultations and interdisciplinary meetings regarding patient treatment planning, progress, and referrals Connect patients with appropriate services, community resources, crisis services, and higher levels of care as clinically indicated Provide crisis intervention and support as needed Maintain timely, accurate, and compliant clinical documentation in accordance with agency policy, state and federal regulations, and funding requirements, including intake documentation, psychosocial assessments, treatment plans, progress notes, service logs, releases of information, and discharge summaries Complete and submit all clinical documentation and progress notes within 48 hours of service delivery Other duties as assigned
