Description
The Chart Integrity Auditor is responsible for overseeing the HIM department's medical records analysis and reporting duties. The Auditor will liaison with providers and leadership to resolve deficiencies/suspensions, and to maintain compliance. The Auditor will perform auditing, scanning, indexing, and release of medical records in accordance with state, federal, and other regulatory guidelines. The Auditor will support HIM administrative tasks as needed.
Responsibilities
Responsible for records analysis and reporting processes
Performs chart analysis on all inpatient, outpatient, and emergency room records to ensure correct documentation and deficiency assignments
Reviews task queues to ensure proper content and that completion standards are met
Generates and manipulate daily reports and create files to support the suspension process
Informs leadership and department employees of daily suspension process notification
Completes daily updates within deficiency spreadsheets
Follows appropriate policies and procedures with regards to medical record completion
Communicates with providers to resolve deficiencies and maintain compliance
Completes records for Joint Commission compliance and delinquent record rate in collaboration with providers and leadership
Assigns, assists, and responds to providers with questions related to deficiency or suspension processes
Prepares and sends out listings of providers deficiencies and provider suspension recommendations
Oversees pools, proxies, and message centers and clarifies declined deficiencies
Monitors and verifies providers have completed their deficiencies within the application
Assists in the reactivation of suspended providers
Performs medical record scanning, indexing, and releases
Completes indexing audits of hospital and ambulatory records
Scans and/or indexes medical records into the electronic medical record
Assists Risk Management in reviewing and preparing medical records for legal inquiries
Utilizes healthcare applications and systems for completion of record requests
Validates requests and releases protected health information from multiple sources per state, federal, and other regulatory agency guidelines
Completes Release of Information forms and Transfer of Care documents to facilitate record release and retrieval
Assists and completes authorizations for walk-in patients
Supports HIM administrative tasks as assigned
Oversees incoming phone calls to department and directs customers to the appropriate area
Assists in training and mentoring team members and HIM externs
Troubleshoots HIM hardware and software
Identifies and troubleshoots simple computer issues and escalates when needed
Qualifications
Education:
High School or GED. At least one or more completed technical courses related to Health Information Management, Medical Terminology, or Business
Experience:
3 years in Health Information Management (HIM)
Skills:
Problem-solving and decision making
Excellent interpersonal skills while understanding the need for a positive customer service experience
Maintaining Quality Control standards
