    "description": "<p>Case Manager/Supports Coordinator-Contractual-Mt. Clemens.<br>Clinton Counseling Center is a Private, Non-Profit Outpatient Substance Abuse &amp; Mental Health<br>Clinic located in Mt. Clemens, MI. The Case Manger provides support to individuals receiving<br>services through our outpatient clinic. This position is contractual, with flexible hours and some<br>local travel in the community. The role of the case manager is to provide support, connection to<br>community resources, and advocacy for persons served.<br>Responsibilities:<br>1. Assessment and reassessment of clients assigned to a caseload.<br>2. Planning and/or facilitating planning using person-centered principles.<br>3. Developing an Individual Plan of Service (IPOS) using the person-centered planning process<br>4. Reviewing and updating the IPOS at least annually or whenever the client wants to review and<br>update.<br>5. Linking to, coordinating with, follow-up, advocacy with other mental health services and<br>community services/supports.<br>6. Brokering with providers of services/supports.<br>7. Assistance with access to entitlements and/or legal representation.<br>8. Coordination with the other care providers.<br>9. Document clinical information in a complete and timely manner, according to accepted</p>\n<p>10. Participate in case reviews as needed.<br>11. Monitoring of services provided to the client.<br>12. Crisis intervention.<br>13. Discharge planning.<br>14. Outreach/networking with other service providers as needed<br>Education/Experience required:<br>1. Bachelor's Degree (Social Work, Counseling or Psychology); Master's Degree preferred (L/LMSW,<br>L/LPC, T/LLP)<br>2. CADC/CAADC preferred, or MCBAP Development Plan required at time of hire<br>3. Experience preferred but not mandatory<br>4. Appropriate supervision provided by fully licensed LPC or LMSW<br>5. Valid Driver's License with own vehicle for use.<br>6. Ability to pass a criminal background check.</p>",
