    "requisitionDescription": "<div><div><p><br></p><p id=\"isPasted\">Provide direct and indirect nursing care to assigned patients. &nbsp;&nbsp;</p><p>Works under the direct and indirect supervision of the Clinical Services Coordinator and/or Medical Director but is independent in the application of advanced nursing knowledge and skills. &nbsp;Manages complex clinical situations for which he/she is responsible.</p><p id=\"isPasted\"><strong>JOB RESPONSIBILITIES:</strong></p><ul><li>Attends morning staff meetings and updates clinic schedule as needed.</li><li>Conducts initial, periodic and unscheduled nursing assessments of participants.</li><li>Attends Interdisciplinary Team (IDT) meetings and shares information about patients&rsquo; condition, problems, needs and priorities.</li><li>Participates in development of plans of care that integrate information from all disciplines.</li><li>Documents changes in participants&rsquo; status and communicates changes to IDT members.</li><li>Reviews documentation of home care worker and home care supervisor visits and reports through the clinical and IDT as needed.</li><li>Provides nursing care according to accepted practice standards.</li><li>Educates participants, families and care givers about the management of medical conditions.</li><li>Identifies emergency situations and initiates therapy as appropriate.</li><li>Respects participants&rsquo; rights and treats patients in accordance with ethical standards.</li><li>Completes all documentation in the medical record and/or electronic health record (EHR) as required. &nbsp;</li><li>Acts only within the scope of his/her authority to practice.</li><li>Other job related duties as assigned.&nbsp;</li></ul><p><br></p></div></div>\n",
