    "requisitionDescription": "<div><p id=\"isPasted\"><strong>Job Purpose</strong>&nbsp;</p><p>The services provided by a Care Coordinator (CC) focus on helping individuals and family develop skills, access resources and manage illness to be successful in the living, working, learning and social environments of their choice. &nbsp;CC&rsquo;s teach, model and practice skills with person(s)-served to promote recovery and wellness.</p><p>&nbsp;<strong>Qualifications</strong>&nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</p><ul><li>A master&rsquo;s or bachelor&rsquo;s degree in a human services field from a college or university included in the US. Department of Education&rsquo;s database of accredited schools at <u>http:/ope.ed.gov/accreditation</u>, OR</li><li>Qualified Addiction Professional (QAP), OR</li><li>Associate of Applied Science degree in Behavioral Health Support as designated by the Department of Mental Health.</li><li>Bachelor&#39;s degree unrelated to human services field with two years of qualifying experience; OR</li><li>Four years of qualifying experience</li></ul><p>Qualifying Experience must include delivering services to individuals with mental illness, substance use disorders or developmental disabilities. &nbsp; Experience must include at least one of the following:</p><ul><li>Providing one-on-one or group services with a rehabilitation/ habilitation and recovery/resiliency focus;</li><li>Teaching and modeling for individuals how to cope and manage psychiatric, developmental, or substance use issues while encouraging the use of natural resources/ supports;&nbsp;</li><li>Supporting individuals in their efforts to find and maintain housing, employment, and/or function appropriately in a family, school, and community;&nbsp;</li><li>Assisting individuals to achieve the goals and objectives on their individualized treatment plan. &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</li></ul><p><strong>Core Job Duties</strong></p><ul><li>Provide education to person(s)-served to learn about and manage mental illness and/ or substance use disorder.</li><li id=\"isPasted\">Provide transportation for person(s)-served in the staff&#39;s personal vehicle or agency vehicle (if available) when medically necessary and appropriate.</li><li>Link person(s)-served with community resources and help them to develop natural support.</li><li>Help person(s)-served to develop or improve interpersonal, social, family, community and independent living skills.</li><li>Complete required documentation in a timely manner.</li><li>Adherence to all applicable evidence-based practice models.&nbsp;</li><li>Demonstrate a working knowledge of diagnostic criteria set for substance use and/or mental disorders.</li><li>Orally communicate information effectively and accurately. &nbsp; &nbsp; &nbsp; &nbsp; &nbsp; &nbsp;</li><li>Assure program operates within the constraints of the agency&rsquo;s certification (CARF, DMH) standards and strategic plan related to clinical care. &nbsp; &nbsp; &nbsp; &nbsp;&nbsp;</li><li>Abide by program and agency policies and procedures.</li><li>Other job duties and special projects as assigned.</li></ul><p><br></p></div>\n",
