    "ExternalDescriptionStr": "<p><span>FlexStaff is seeking a&nbsp;Registered Nurse with CHHA or hospital experience for our client, a CHHA agency. This is a full-time, Monday-Friday, either 9a-5pm or 8a-4pm, depending on needs.&nbsp;</span></p><p><span><strong>Qualifications:</strong></span></p><ul><li><span>License and current registration to practice as a Registered Professional Nurse in New York State.</span></li><li><span>Graduate of an accredited school of nursing, BSN preferred.</span></li><li><span>A minimum of one year experience in hospital and or minimum of one year experience in a CHHA preferred.</span></li></ul><p><span>You will be responsible for providing and coordinating multidisciplinary health services for patients in the community. Works with the physician and other team members in implementation of plan of care.</span></p><p><span><strong>Duties and Responsibilities:</strong></span></p><ul><li><p style=\"margin-left: 0.25in;\"><span>Coordinates cost-effective patient care that meets insurance and physician approval.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Verifies insurance plan details and checks patient ID cards at home.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Accepts case assignments based on client needs, regardless of location.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Handles all CHHA requirements, including assessments and daily documentation.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Travels to patients' homes/facilities to provide direct care.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Admits patients promptly as needed.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Completes Oasis in-person, accurately reflecting patients' clinical status, including diagnosis coding.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Collaborates with the Director and physician to determine primary diagnoses.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Participates in meetings to report service adherence and patient progress for discharge planning.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Communicates with PT/OT to ensure compliance with physician-ordered therapy.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Serves as the coordinator for the multidisciplinary care team.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Develops care plans with input from health team members, focusing on patient and family goals.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Documents all services provided.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Monitors care plan effects and updates the team on any changes in patient needs.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Adjusts care goals and interventions as necessary.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Educates patients and families on disease management, self-care, and prevention.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Initiates and maintains communication for optimal patient care.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Supervises health and personal care workers via home visits.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Reviews and updates the HHA plan of care with workers and families.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Communicates regularly with HHAs regarding care plans and patient goals.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Documents initial and bi-weekly HHA service supervision.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Plans and documents patient discharges from the program.&nbsp;</span></p></li><li><p style=\"margin-left: 0.25in;\"><span>Manages an assigned caseload in Manhattan. &nbsp;</span></p></li></ul><p><span>Salary Range: $49.7584 per hour</span></p><p><span>*Additional Salary Detail</span></p><p><span>The salary range and/or hourly rate listed is a good faith determination of potential base compensation that may be offered to a successful applicant for this position at the time of this job advertisement and may be modified in the future.
