The primary goal of this role is to support residents in managing the challenges of aging, illness, and long-term care living while promoting the highest possible quality of life.</p><hr><h2><strong>Key Responsibilities</strong></h2><ul><li>Assess and address residents&rsquo; psychosocial and emotional needs</li><li>Conduct admission psychosocial assessments</li><li>Educate residents and families on advance directives and resident rights</li><li>Provide counseling, support, and case management services</li><li>Document care plans, interventions, and progress notes</li><li>Participate in interdisciplinary care planning meetings</li><li>Complete MDS 3.0 assessments in a timely manner</li><li>Collaborate with families, caregivers, and community resources</li><li>Coordinate discharge planning and transitions of care</li><li>Facilitate referrals to home care and outpatient services</li><li>Communicate with insurance providers regarding coverage</li><li>Support family meetings and support groups</li><li>Provide education to staff and families</li><li>Serve as designee for Director of Social Services as needed</li></ul><hr><h2><strong>Qualifications</strong></h2><ul><li>Bachelor&rsquo;s Degree in Social Work (BSW) required; MSW preferred</li><li>Knowledge of Alzheimer&rsquo;s disease, dementia, and aging populations</li><li>Strong communication and interpersonal skills</li><li>Ability to work in a collaborative, team-based environment</li><li>Compassionate, patient, and professional demeanor</li><li>Strong organizational and documentation skills</li><li>Computer proficiency, including EMR systems</li></ul><hr><h2><strong>Work Environment &amp; Physical Requirements</strong></h2><ul><li>Ability to move throughout the facility and interact with residents regularly</li><li>Ability to sit, stand, and bend for extended periods</li><li>Occasional lifting (up to 50 lbs)</li><li>Ability to manage the emotional demands of the role</li></ul><hr><h2><strong>Why Join The Osborn?</strong></h2><ul><li>Supportive, mission-driven environment</li><li>Collaborative interdisciplinary team</li><li>Opportunity to make a meaningful impact in residents&rsquo; lives</li></ul></div>\n",
