Weekdays with occasional weekend rotation - various schedule options available for discussion</span></p><p><span style=\"font-size: 16px;\"><strong><span style=\"color: rgb(41, 105, 176);\">What You&rsquo;ll Do</span></strong></span></p><p><span style=\"font-size: 16px;\"><strong>Homecare Admissions</strong></span></p><ul type=\"disc\"><li style=\"font-size: 16px;\">Conduct comprehensive hospice admission assessments</li><li style=\"font-size: 16px;\">Evaluate eligibility and collaborate with the hospice physician</li><li style=\"font-size: 16px;\">Educate patients and families about hospice philosophy and services</li><li style=\"font-size: 16px;\">Establish an initial plan of care focused on comfort and symptom management</li><li style=\"font-size: 16px;\">Reconcile medications and coordinate urgent needs (DME, medications)</li><li style=\"font-size: 16px;\">Communicate clearly with the interdisciplinary team and primary providers</li></ul><p><span style=\"font-size: 16px;\"><strong>Hospital Transitions</strong></span></p><ul type=\"disc\"><li style=\"font-size: 16px;\">Coordinate hospice admissions from hospital to home</li><li style=\"font-size: 16px;\">Perform hospital visits and support seamless discharge planning</li><li style=\"font-size: 16px;\">Order DME and assess medication needs prior to discharge</li><li style=\"font-size: 16px;\">Collaborate with Referral Center &amp;
